Who we are
Wellchosen Health is an independent consumer guide at wellchosenhealth.com. [Business legal name and mailing address will be added here] You can reach us at contact@wellchosenhealth.com. More about us is on our about page.
Information we collect
When you read the site
Our hosting service receives standard request data, such as IP address, browser type, the page requested and the time, so it can deliver pages and protect the site from abuse. We do not combine it with other data or use it to profile readers. We do not run analytics or advertising tools that follow you around the web.
When a practice sends the clinic inquiry form
The clinic inquiry form on sponsor a city and our contact page collects a practice name, contact name, email, phone, the city of interest and the services the practice offers. We use it only to reply about Sponsored placements and to check that a practice meets our requirements. The form sends what you enter by email to our inbox, and the form handler does not log it.
When you email us
We receive your email address and whatever you write. We use it to answer you, to handle corrections and to respond to data requests. Please do not send health details.
Cookies
Our pages do not set advertising or analytics cookies. If that ever changes, we will update this policy first.
Links to other websites
Our pages link to drugmakers, government sites, telehealth companies and clinics. Some telehealth links are affiliate links, explained on how we make money. When you leave our site, the other site's privacy policy applies. A company you reach through an affiliate link may record that you came from Wellchosen Health so it can pay a commission; we do not send it your name, email or any health information.
How we share information
- With the service providers that host the site and handle our email, only so they can provide those services.
- With one chosen practice, once a patient request form opens, and only with your consent. See our consumer health data policy.
- When the law requires it, or to protect the site and its users from fraud or abuse.
We do not sell personal data and do not share it for advertising.
How long we keep it
We keep inquiry and email messages as long as we need them to answer, to keep a record of a sponsorship, or to meet legal duties, and then delete them.
Children
This site is for adults. We do not knowingly collect personal data from children under 13. If you think a child has sent us personal data, email us and we will delete it.
Your requests
You can ask to see, correct or delete personal data we hold about you. Email contact@wellchosenhealth.com with the subject "Data request." We will confirm we received it and reply with what we hold and what we did.
Changes
When this policy changes, the date at the top of the page changes too. See also our terms of use and contact page.
