Mounjaro for the 13.9% with diabetes

In Bexar County, 13.9% of adults report a diabetes diagnosis and 34.1% have obesity, per CDC's 2023 estimates [3]. For the first group, the relevant brand is usually Mounjaro, which is approved for type 2 diabetes and, since August 2026, for lowering the risk of major cardiovascular events in high-risk adults with type 2 diabetes [6]. Its list price is $1,112.16 for four pens [7].

  • Plan covers Mounjaro: the savings card can lower the cost to $25 for a 1-, 2- or 3-month fill, with savings capped at $150 for a one-month fill [8].
  • Plan does not cover it: the card can bring a month to as low as $499 [8].
  • No insurance, Medicare or Medicaid: the card cannot be used [8].

Zepbound: LillyDirect or a savings card

Two cash routes to Zepbound for San Antonio patients (seen Oct 17, 2026)
StrengthLillyDirect self-payKwikPen with savings card, plan does not coverRef
2.5 mg$299$299[1][2]
5 mg$399$399[1][2]
7.5 to 15 mg$449 if refilled within 45 days$449, same 45-day rule[1][2]
The card route is for commercially insured patients, allows up to 11 fills a year, and ends 12/31/2026. Pen needles are extra.

LillyDirect ships free or hands off at a Walmart Pharmacy [1]. The card route lets you use another pharmacy, but only if you have commercial insurance [2]. See Zepbound insurance and savings and the tirzepatide cost guide.

Sleep apnea: the other Zepbound approval

FDA approved Zepbound on December 20, 2024 for moderate to severe obstructive sleep apnea in adults with obesity, the first drug for that use [9]. In the label's two 52-week trials, the apnea-hypopnea index fell by about 25 to 29 events an hour on Zepbound versus about 5 on placebo [10]. That approval lets Medicare Part D plans cover Zepbound for sleep apnea. Since 13.4% of San Antonians are 65 or older [4], it is worth knowing that sleep apnea patients stay in their regular Part D plan rather than the GLP-1 Bridge [11].

What trials found on weight

Head to head against semaglutide in SURMOUNT-5, tirzepatide users lost 20.2% of body weight on average at 72 weeks, compared with 13.7%, and waistlines shrank 18.4 cm versus 13.0 cm [12]. Stomach side effects were the most common complaint in both groups. Averages are not a forecast for any one patient. More on Wegovy versus Zepbound.

Compounded offers after the court rulings

Compounding pharmacies and their allies sued over FDA's decision that the tirzepatide shortage was over. On August 27, 2026, the Fifth Circuit affirmed FDA's win [13]. FDA says outsourcing facilities cannot compound tirzepatide from bulk ingredients now that it is off the shortage list, and pharmacy compounding is limited to patient-specific needs [14]. Compounded tirzepatide is not FDA-approved [15]. See compounded tirzepatide.

Before your first dose

Tell the prescriber about any family history of medullary thyroid carcinoma or MEN 2; the label's boxed warning rules tirzepatide out in that case. Stop and call if severe stomach pain persists, a possible sign of pancreatitis, or if an allergic reaction appears. Insulin and sulfonylureas raise the risk of low blood sugar. Tell anesthesia staff you take it before any procedure with sedation [10]. Call 911 in an emergency.

Check a prescriber with the Texas Medical Board [16]; Texas Medicaid does not pay for obesity GLP-1s [5]. Other pages: tirzepatide guide, finding tirzepatide near you, Austin, Houston, Dallas.

Questions people ask

How much is Zepbound in San Antonio without insurance?

LillyDirect listed $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5 to 15 mg (with a refill within 45 days) per 28-day fill on October 17, 2026 [1].

My insurance denied Zepbound. Can a savings card help?

Yes, if you have commercial insurance. Lilly's card lets patients whose plan does not cover Zepbound pay $299, $399 or $449 for the KwikPen by dose, up to 11 fills a year, through December 31, 2026 [2].

What does Mounjaro cost with a savings card?

With coverage, as little as $25 for a 1-, 2- or 3-month fill; without coverage, as low as $499 a month. Uninsured and government-plan patients cannot use it [8].

Does Medicare cover Zepbound for sleep apnea?

Part D plans can cover Zepbound for obstructive sleep apnea because of its December 2024 approval; sleep apnea patients use their regular Part D plan, not the GLP-1 Bridge [9][11].

Did tirzepatide beat semaglutide in a head-to-head trial?

In SURMOUNT-5, average weight loss at 72 weeks was 20.2% with tirzepatide and 13.7% with semaglutide [12].

Can San Antonio pharmacies still make compounded tirzepatide?

Only in limited patient-specific cases, per FDA, and outsourcing facilities cannot compound it from bulk. The Fifth Circuit upheld FDA's shortage decision on August 27, 2026 [14][13].
