Tirzepatide is a once-weekly injected medicine that activates the GIP and GLP-1 receptors, sold as Zepbound for weight management and sleep apnea and as Mounjaro for type 2 diabetes.
How tirzepatide works in the body
The FDA label describes tirzepatide as a GIP receptor and GLP-1 receptor agonist. It binds the same receptors as the body's own GIP and GLP-1. The label calls GLP-1 a physiological regulator of appetite and calorie intake, and says animal work suggests GIP may add to the effect on food intake. Both receptors are found in brain areas that control appetite [1]. The drug also slows how fast the stomach empties [1].
So the label frames tirzepatide as a drug that lowers how much people eat. It does not describe it as burning fat directly. It does say the weight lost is more fat than lean mass [1]. In a SURMOUNT-1 substudy, about three quarters of the weight lost was fat [5].
When it kicks in, and when weight loss shows
Two different clocks run at once. The first is drug level. After each shot, tirzepatide peaks in the blood at a median of 24 hours, and steady levels are reached after 4 weeks of weekly doses [1]. About 80% of each injected dose reaches the bloodstream [1].
The second clock is the dose schedule. Everyone starts at 2.5 mg once weekly for 4 weeks. The label says 2.5 mg is for starting treatment and is not a maintenance dose. The first maintenance dose, 5 mg, begins in week 5, with further 2.5 mg steps no sooner than every 4 weeks [6]. The trials reported their main results at 72 weeks, and the sources in our research do not give week-by-week averages, so this page does not forecast how much you will lose by a given week. See tirzepatide dosage for the full schedule.
How much weight people lost in the trials
| Trial | Who took part | Group | Average weight change | Length |
|---|---|---|---|---|
| SURMOUNT-1 [2] | Adults without diabetes | 5 mg | -15.0% | 72 weeks |
| SURMOUNT-1 [2] | Adults without diabetes | 10 mg | -19.5% | 72 weeks |
| SURMOUNT-1 [2] | Adults without diabetes | 15 mg | -20.9% | 72 weeks |
| SURMOUNT-1 [2] | Adults without diabetes | Placebo | -3.1% | 72 weeks |
| Zepbound Study 2 [1] | Adults with type 2 diabetes | 10 mg / 15 mg / placebo | -12.8% / -14.7% / -3.2% | 72 weeks |
| SURMOUNT-5 [7] | Head-to-head vs semaglutide | Tirzepatide vs semaglutide | -20.2% vs -13.7% | 72 weeks |
The spread matters more than the average. In SURMOUNT-1, 85%, 89% and 91% of people on the three doses lost at least 5% of their weight, vs 35% on placebo. On 10 mg and 15 mg, 50% and 57% lost at least 20%, vs 3% on placebo [2]. Put the other way, roughly 1 in 10 people on tirzepatide did not reach 5%.
Why am I not losing weight on tirzepatide?
- You may still be on the starting dose. The label says 2.5 mg is not meant for maintenance [6]. Lower "microdose" plans have no trial data; see microdosing tirzepatide.
- Averages hide slow responders. 9% to 15% of people on tirzepatide in SURMOUNT-1 lost less than 5% [2].
- The drug was tested with lifestyle changes. Zepbound is approved for use with a reduced-calorie diet and more physical activity [6]. See what to eat on tirzepatide.
- Compounded vials are not FDA-checked. FDA does not review compounded drugs for safety, effectiveness or quality [8], so the strength in a compounded vial has not been confirmed by FDA.
None of our sources studies weight gain on tirzepatide or how to break a plateau. If the scale is moving the wrong way, ask your prescriber whether a dose change fits the label's schedule, which tops out at 15 mg weekly [6].
Does compounded or oral tirzepatide work?
The trial results on this page come from FDA-approved Zepbound and Mounjaro. Compounded tirzepatide is not FDA-approved and was not tested in those trials [8]. Since the shortage ended, FDA limits pharmacies to patient-specific prescriptions and bars routine copies of approved drugs [9]. Read compounded tirzepatide before you buy.
No oral, sublingual or drop form of tirzepatide is FDA-approved; both approved brands are injections [6] [10]. The approved GLP-1 pills are different molecules, covered in Foundayo vs Wegovy pill. Our oral tirzepatide page explains what online pills and drops are.
Does it keep working, and how long can you take it?
SURMOUNT-4 followed people for 88 weeks. After a 36-week open-label lead-in in which people lost an average of 20.9%, those who stayed on tirzepatide lost a further 5.5%, while those switched to placebo regained 14.0% [11]. Over the full 88 weeks, the group that continued lost 25.3% vs 9.9% for the group that stopped [11]. Zepbound's indication covers keeping weight off long term [6]. More on coming off it in stopping tirzepatide.
How long tirzepatide stays in your system
The elimination half-life is about 5 to 6 days [1]. Blood levels fall by about half every 5 to 6 days after the last dose, so the drug clears over several weeks, not a few days.
Is it safe for weight loss, and who can take it?
FDA approved Zepbound for adults with a BMI of 30 or more, or 27 or more with a weight-related condition [4]. Its label says safety and effectiveness have not been established in pediatric patients, so a 16-year-old is outside the Zepbound label [1]. Mounjaro is approved for type 2 diabetes from age 10, not for weight loss [10]. Both carry a boxed warning about thyroid C-cell tumors seen in rats [1]. See tirzepatide side effects.
Tirzepatide vs semaglutide
In SURMOUNT-5, people on tirzepatide lost an average of 20.2% at 72 weeks vs 13.7% on semaglutide, and waist size fell 18.4 cm vs 13.0 cm [7]. The abstract reports results at 72 weeks, not which drug acts sooner. Compare costs and labels in Wegovy vs Zepbound.
What staying on it costs
Results depend on months of treatment, so price matters. On LillyDirect, Zepbound costs $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5 to 15 mg when refilled within 45 days, checked October 17, 2026 [12]. Full comparisons are on tirzepatide cost and getting tirzepatide online.
Questions people ask
How long does it take for tirzepatide to work?
How much weight can you lose on tirzepatide?
In SURMOUNT-1, adults without diabetes lost an average of 15.0% (5 mg), 19.5% (10 mg) and 20.9% (15 mg) of body weight at 72 weeks, vs 3.1% on placebo [2]. Individual results ranged widely.
How does tirzepatide work for weight loss?
It activates GIP and GLP-1 receptors, including in brain areas that control appetite, and slows stomach emptying. The FDA label describes the effect as lowering appetite and food intake [1].
How long does tirzepatide stay in your system?
Its half-life is about 5 to 6 days [1], so blood levels fall by about half every 5 to 6 days and the drug clears over several weeks after the last shot.
Why am I not losing weight on tirzepatide?
Does compounded tirzepatide work?
It has not been tested in the trials behind Zepbound and Mounjaro. Compounded tirzepatide is not FDA-approved, and FDA does not review it for safety, effectiveness or quality [8].
How long has tirzepatide been around?
How much tirzepatide should you start with?
The label starting dose is 2.5 mg once weekly for 4 weeks, then 5 mg. The 2.5 mg dose is for starting only, not maintenance [6]. Your prescriber sets your dose.
Do you lose muscle on tirzepatide?
Some. In a SURMOUNT-1 substudy, fat mass fell 33.9% and lean mass 10.9% at 72 weeks, so about a quarter of the weight lost was lean mass [5].
Does tirzepatide work better than semaglutide?
In the SURMOUNT-5 head-to-head trial, average weight loss at 72 weeks was 20.2% on tirzepatide vs 13.7% on semaglutide [7].
