Getting tirzepatide online means a licensed clinician evaluates you by video or questionnaire, writes a prescription, and a pharmacy ships brand Zepbound or a compounded tirzepatide product to your home.

Do you need a prescription for tirzepatide?
Yes. Every legal form of tirzepatide in the US is prescription-only. That covers Zepbound for weight and sleep apnea, Mounjaro for type 2 diabetes, and compounded tirzepatide made by a pharmacy for one patient [1] [6] [7]. Even TrumpRx, the federal price portal, requires a valid prescription and routes Zepbound buyers to LillyDirect [8] [9].
Sites that sell tirzepatide with no prescription usually label it "research use only." FDA has sent warning letters to sellers of those products and says their quality is unknown and may be harmful [5]. Our guide to research-use-only GLP-1s explains what those vials are and why they are not a shortcut.
Who can prescribe tirzepatide
The Zepbound and Mounjaro labels do not limit prescribing to a specialty [1] [6]. Your primary care doctor, an OB-GYN, an endocrinologist or a telehealth clinician can prescribe it if they judge it right for you. Zepbound is approved for adults with obesity, or overweight with at least one weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity [1]. Mounjaro is approved for type 2 diabetes, so using it for weight alone is outside its label [6]. See Mounjaro vs Zepbound for the full difference.
People ask about two specific prescribers. Midi Health, a menopause telehealth clinic, says its care plans may include GLP-1s; it charges $250 for a self-pay first visit and $150 for a return visit, checked Oct 17, 2026 [10]. We did not confirm whether Midi prescribes tirzepatide in particular, so ask before you book. We also did not confirm VA formulary rules for tirzepatide; ask your VA pharmacy or primary care team.
How ordering tirzepatide online works
- Intake. You fill out a health history, usually with height, weight and current medicines. Many programs charge a membership or visit fee here.
- Clinician review. A licensed prescriber decides whether tirzepatide fits. Brand Zepbound is labeled for adults who meet its indication [1].
- Prescription goes to a pharmacy. For brand Zepbound, the prescriber can send it to LillyDirect Pharmacy, which calls you to confirm details and offers free home delivery or pickup at Walmart Pharmacy [2]. For compounded tirzepatide, the program sends it to a compounding pharmacy.
- Refills. Each refill needs an active prescription. On LillyDirect, the $449 price for 7.5 mg and above applies only if you refill within 45 days of the last fill [2].
If your own doctor will prescribe, you can skip the telehealth fee entirely and have the prescription sent to LillyDirect. That is often the cheapest path. Our cheapest tirzepatide comparison shows the first-year math.
Brand Zepbound vs compounded tirzepatide
These are different products. Zepbound is FDA-approved and made by Eli Lilly. Compounded tirzepatide is not FDA-approved, and FDA does not review it for safety, effectiveness or quality [5]. FDA ended its shortage-era tolerance for compounded tirzepatide in 2025 (March 5 for 503A pharmacies and March 19 for 503B outsourcing facilities) [7]. Today a 503A pharmacy may compound it only for an individual patient, and copies of an approved drug are barred unless the prescriber documents a change that makes a significant difference for that patient [7].
FDA reported more than 730 adverse event reports for compounded tirzepatide as of May 31, 2026, and says the true number is likely higher because 503A pharmacies do not have to report [5]. Read compounded tirzepatide: cost, legality and safety before choosing it.
What to check before you pay any online program
- The prescriber. A real clinician should review your history. Programs that promise approval before review are selling, not prescribing.
- The pharmacy. Brand drugs should come from LillyDirect, NovoCare or a normal pharmacy. For compounded drugs, ask which pharmacy compounds it. FDA says a telehealth company that puts its own name on the vial implies it is the compounder, which it cited as misleading [11].
- FDA letters. Search the company on our FDA warning letter tracker. FDA cites claims like "generic Zepbound" and "same active ingredient" as misleading for compounded drugs [12] [13].
- The full price. Ask for the price at your maintenance dose, not the intro price, plus membership, consult, lab and shipping fees.
- Cancellation. Read the refund terms. Several programs bill multi-month plans up front or charge fees to cancel.

Using insurance or Medicare online
If your commercial plan covers Zepbound, Lilly's savings card can bring a fill to as little as $25, up to $100 off a month and $1,300 a year; the card ends Dec 31, 2026 and cannot be used by Medicare or Medicaid members [42]. Programs such as Ro, Found and Knownwell help with prior authorizations [15] [43].
Medicare Part D members who qualify can get the Zepbound KwikPen through the Medicare GLP-1 Bridge for a $50 monthly copay from July 1, 2026 through Dec 31, 2027; single-dose Zepbound vials and pens are not covered [44] [45]. Details on GLP-1 coverage and Medicare.
Reading your prescription and pharmacy label
Brand Zepbound comes in fixed doses of 2.5 to 15 mg, so the label names the dose directly [1]. The label schedule starts at 2.5 mg weekly for 4 weeks, then raises the dose in 2.5 mg steps no sooner than every 4 weeks, up to 15 mg [1]. Compounded vials are different: the number of units for a given milligram dose depends on the concentration printed on the pharmacy label. Follow your prescriber's instructions and that label exactly, and ask the pharmacist if anything is unclear. See tirzepatide dosage for the label schedule.
Safety facts from the Zepbound label
Zepbound has a boxed warning for thyroid C-cell tumors seen in rats; do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2 [46]. Stop and get care for severe stomach pain that will not go away, which can signal pancreatitis, and for signs of a serious allergic reaction [46]. Low blood sugar is more likely with insulin or a sulfonylurea [46]. Stop Zepbound when pregnancy is recognized [46]. In an emergency, call 911.
