Zepbound is Eli Lilly's brand of tirzepatide, a once-weekly injection approved for chronic weight management and for moderate to severe obstructive sleep apnea in adults with obesity [3].

What you will pay for Zepbound
The price depends on three things: the dose, whether you refill on time, and whether a commercial plan covers the drug. LillyDirect charges the same for the single-dose vial and the four-dose KwikPen [1]. Lilly defines a LillyDirect month as 28 days, so a year is 13 fills [1]. For every GLP-1 in one table, see our GLP-1 Price Tracker.

Hidden costs to plan for
- The step-up schedule. The label starts everyone on 2.5 mg for 4 weeks, then 5 mg, then 2.5 mg steps after at least 4 weeks on each dose [3]. On LillyDirect that moves you from $299 to $399 to $449 a month.
- A late refill. On 10 mg or higher, refilling after day 45 turns a $449 month into a $699 month [1].
- Needles and syringes. The KwikPen needs pen needles, sold separately. The vial needs a needle and syringe [1]. The label says to use a new syringe and needle for each injection [3].
- The prescriber. LillyDirect sells only with a prescription. If you get it through a telehealth program, add its fee, such as Ro's $74 to $149 monthly membership [7].
- The yearly total. Thirteen fills at $449 come to $5,837 a year before any visit or membership fees.
Ways to pay less
- Check your plan first. If a commercial plan covers Zepbound, the savings card can cut the copay to $25 [2]. See how plans decide on Zepbound coverage.
- Refill on time. The $449 price for the higher doses holds only when each fill comes within 45 days of the last one [1].
- Pick the form you prefer. The vial and KwikPen cost the same on LillyDirect [1]. The KwikPen holds four weekly doses [9].
- Do not expect TrumpRx to beat LillyDirect. Its Zepbound page sends buyers to LillyDirect [6]. Our TrumpRx guide explains what it does.
- Compare other drugs. Wegovy is $349 a month on NovoCare after its intro offer [10], and Foundayo starts at $149 [11].
Compounded tirzepatide is not Zepbound
Compounded tirzepatide is not FDA-approved, and FDA does not review it for safety, effectiveness or quality [12]. FDA ended its enforcement leeway for pharmacies compounding tirzepatide in March 2025 after the shortage ended [13]. Read compounded tirzepatide: cost, legality and safety before you buy.
Insurance and Medicare
Zepbound is approved for adults with obesity, or overweight with at least one weight-related condition, and for moderate to severe sleep apnea in adults with obesity [3]. Whether your plan pays is a benefit decision, so two people with the same insurer can get different answers. The savings card cannot be used by people on Medicare or Medicaid [2].
Since July 1, 2026, the Medicare GLP-1 Bridge covers the Zepbound KwikPen for weight management at a $50 copay, through Dec 31, 2027. It does not cover Zepbound vials or single-dose pens [14]. People with moderate to severe sleep apnea are not eligible for the Bridge, and Medicare says their regular drug plan might cover a GLP-1 [5]. Full detail: does Medicare cover GLP-1s.
How to get Zepbound, step by step
- Get a prescription. Zepbound is prescription-only [1]. Your own clinician or an online program can write it.
- Have the prescriber send it to LillyDirect Pharmacy. A partner pharmacy then contacts you to confirm details and savings [1].
- Choose free home delivery or free pickup at Walmart Pharmacy [1].
- Refill within 45 days to keep the $449 price on the higher doses [1].

On results: in the SURMOUNT-1 trial, people without diabetes on the 15 mg dose lost an average of 20.9% of body weight at 72 weeks, against 3.1% on placebo [19]. In the SURMOUNT-5 head-to-head trial, the tirzepatide group lost an average of 20.2% at 72 weeks, against 13.7% for semaglutide [20]. Averages are not a promise for any one person.

Before you start Zepbound
Zepbound carries a boxed warning: tirzepatide caused thyroid C-cell tumors in rats, and it is not known whether it does in people. Do not use it if you or a family member had medullary thyroid carcinoma or MEN 2 [22]. Stop it and call your prescriber if you have severe stomach pain that will not go away, which can be a sign of pancreatitis, or signs of an allergic reaction. Low blood sugar is more likely if you also take insulin or a sulfonylurea. The label says to stop Zepbound when pregnancy is recognized [22]. For an emergency, call 911.
Questions people ask
How much is Zepbound without insurance?
On LillyDirect, Zepbound costs $299 a month for 2.5 mg, $399 for 5 mg and $449 for 7.5 to 15 mg when you refill within 45 days. Refill later and 7.5 mg costs $499 while 10 to 15 mg cost $699 (Lilly, checked Oct 17, 2026) [1].
How does the Zepbound savings card (coupon) work?
If a commercial plan covers Zepbound, the card can lower the copay to $25 for a 1-, 2- or 3-month fill, with savings capped at $100 per month and $1,300 a year. If the plan does not cover it, the card brings the single-dose pen to $499 a month. Medicare and Medicaid members cannot use it, and card savings end Dec 31, 2026 [2].
How much is Zepbound on LillyDirect, and is the vial cheaper than the pen?
LillyDirect charges $299 to $449 a month depending on dose, and the single-dose vial and the KwikPen cost the same. Delivery and Walmart Pharmacy pickup are free (checked Oct 17, 2026) [1].
Is there a generic for Zepbound?
Is compounded tirzepatide as effective as Zepbound?
No one can say. Compounded tirzepatide is not FDA-approved and FDA does not review it for safety, effectiveness or quality [24]. FDA had received more than 730 adverse event reports for compounded tirzepatide by May 31, 2026 [12]. Zepbound's results come from trials such as SURMOUNT-1, where the 15 mg group lost an average of 20.9% of body weight at 72 weeks [19].
Does insurance cover Zepbound?
It depends on your plan's drug list, not the insurer's name. Zepbound is approved for weight management and for sleep apnea with obesity [3]. If your commercial plan covers it, the savings card can bring the copay to $25 [2]. Medicare covers the KwikPen at $50 a month under the GLP-1 Bridge for people who qualify [5].
