What Zepbound is
Zepbound contains tirzepatide, a GIP receptor and GLP-1 receptor agonist made by Eli Lilly [2]. So yes, it is a GLP-1 medicine, and it also acts on a second hormone receptor, GIP. It is not semaglutide, the drug in Wegovy and Ozempic, and it is not insulin. It is a brand product, not a compounded one; compounded tirzepatide is a different, unapproved product (see compounded tirzepatide).
Is Zepbound the same as Mounjaro? Same molecule, different labels. Mounjaro is approved for type 2 diabetes; Zepbound for weight and sleep apnea. Lilly says Zepbound should not be used with other tirzepatide products [3]. Full comparison: Mounjaro vs Zepbound. Drug background: tirzepatide.
What Zepbound is approved to treat
- Weight. With a reduced-calorie diet and more activity, to reduce excess weight and keep it off in adults with obesity, or overweight with at least one weight-related condition [1]. FDA described this as a BMI of 30 or more, or 27 or more with at least one weight-related condition [4].
- Obstructive sleep apnea. Moderate to severe sleep apnea in adults with obesity. FDA called it the first drug for certain patients with the condition [5]. In the two label trials, breathing interruptions fell by 25.3 and 29.3 events per hour, vs about 5 on placebo [2].
How Zepbound works
The label says tirzepatide lowers calorie intake, likely through appetite, and slows stomach emptying, most after the first dose [2]. Does Zepbound burn fat? It works by eating less, not by burning fat directly; the label notes weight loss on tirzepatide includes more fat than lean mass [2]. More in how GLP-1 medicines work.
Pens, vials and the KwikPen
| Form | Doses | Notes |
|---|---|---|
| Single-dose pen | 2.5, 5, 7.5, 10, 12.5 or 15 mg per 0.5 mL | One shot, then discard [1] |
| Single-dose vial | Same strengths | Needs a syringe that measures 0.5 mL; new syringe and needle each time [1] |
| KwikPen (multi-dose) | 4 doses per pen, 2.5 to 15 mg per dose | Launched Feb 23, 2026 [7]; pen needles sold separately [6]; never share a pen [1] |
Doses and when to take it
Everyone starts at 2.5 mg once a week for 4 weeks, a starting dose only. Then 5 mg, with 2.5 mg increases after at least 4 weeks on each dose. Maintenance is 5, 10 or 15 mg for weight and 10 or 15 mg for sleep apnea; 15 mg is the maximum [1]. Take it any day and time, with or without food, as long as doses stay at least 72 hours apart; a missed dose can be taken within 4 days [1]. Details: Zepbound dosage and how to use tirzepatide.
How safe is Zepbound?
| Side effect | Placebo | 5 mg | 10 mg | 15 mg |
|---|---|---|---|---|
| Nausea | 8 | 25 | 29 | 28 |
| Diarrhea | 8 | 19 | 21 | 23 |
| Vomiting | 2 | 8 | 11 | 13 |
| Constipation | 5 | 17 | 14 | 11 |
| Injection site reactions | 2 | 6 | 8 | 8 |
| Hair loss | 1 | 5 | 4 | 5 |
The boxed warning covers thyroid C-cell tumors in rats; whether this happens in humans is unknown. Zepbound is not for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 [2]. Other warnings: severe stomach reactions (not recommended in severe gastroparesis), kidney injury from fluid loss, gallbladder disease, pancreatitis, allergic reactions, diabetic eye complications, and aspiration during anesthesia [2][1]. In February 2026 the suicidal thoughts warning was removed from the label [1].
Can Zepbound cause low blood sugar?
Rarely without diabetes drugs. In the trial of adults without diabetes, 0.3% had blood sugar below 54 mg/dL vs 0% on placebo. In adults with type 2 diabetes it was 4.2% vs 1.3%, rising to 10.3% for people also taking a sulfonylurea [2]. Is Zepbound good for diabetics? It is labeled for weight, not blood sugar; in that diabetes trial, average weight loss was 12.8% to 14.7% [2]. The diabetes-labeled version is Mounjaro.
Pregnancy, birth control and other medicines
"Weight loss offers no benefit to a pregnant patient and may cause fetal harm," so Zepbound is stopped when pregnancy is recognized [2]. Birth control pills may work less well: use a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase [8]. See Zepbound interactions.
Call your clinician or 911
Stop Zepbound and get care for severe stomach pain that will not go away (possible pancreatitis) [2]. Get emergency help for swelling of the face, lips or throat or trouble breathing. Report a neck lump, trouble swallowing or lasting hoarseness. Tell your surgical team you take it before any procedure with anesthesia [2].
Results
In the main weight trial, average loss at 72 weeks was 15.0% on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg, vs 3.1% on placebo [2]. In SURMOUNT-4, people who switched to placebo after 36 weeks regained 14.0% of body weight by week 88 [9]. Read Zepbound results and what happens when you stop.
