Tirzepatide storage rules are the refrigeration, room-temperature, light and freezing limits set in Section 16.2 of the FDA label and repeated in the Patient Information.
- Refrigerator
- 36°F to 46°F (2°C to 8°C) [1]
- Single-dose pen or vial out of the fridge
- Up to 86°F (30°C) for a total of 21 days [1]
- KwikPen or multi-dose vial
- Discard after 30 days at room temperature, 30 days after first use, or 4 doses [1]
- Freezing
- Do not freeze; do not use if frozen [1]
- Compounded vials
- Follow the pharmacy label; FDA advises discarding 28 days after first use [2]
The label's storage rules, by product
| Product | Refrigerator | Without refrigeration | When to throw it away |
|---|---|---|---|
| Single-dose pen | 36°F to 46°F (2°C to 8°C) | If needed, up to 86°F (30°C) for a total of 21 days | After 21 days total at room temperature |
| Single-dose vial | 36°F to 46°F (2°C to 8°C) | If needed, up to 86°F (30°C) for a total of 21 days | After 21 days total at room temperature |
| KwikPen (4 doses) | 36°F to 46°F (2°C to 8°C) | Or room temperature up to 86°F (30°C) | After 30 days at room temperature, 30 days after first use, or 4 weekly doses, even if medicine is left |
| Multi-dose vial (4 doses) | 36°F to 46°F (2°C to 8°C) | Or room temperature up to 86°F (30°C) | After 30 days at room temperature, 30 days after first use, or 4 weekly doses, even if medicine is left |
For every form, keep the medicine in its original carton to protect it from light, never freeze it, and keep it out of reach of children [1]. Mounjaro is the same molecule under a separate label, covered on Mounjaro storage. Brand overview: Zepbound.
How long it keeps, opened and unopened
The room-temperature clock is a running total. A single-dose pen that spent 5 days out of the fridge has 16 of its 21 days left, even if you put it back in. A KwikPen or multi-dose vial has a second clock that starts at first use: 30 days or 4 doses, whichever comes first [1].
The storage section gives no separate limit for unopened product kept in the fridge, so the expiration date on the carton is the date to watch. It also gives no guidance on using tirzepatide after that date. Do not count on expired product still being good; ask your pharmacist about a replacement.
If it gets warm, hot or left out
Up to 86°F (30°C), unrefrigerated time is allowed within the limits in the table, and it all counts toward the total [1]. The label gives no allowance for temperatures over 86°F. If a pen or vial got hotter than that, for example in a parked car, call your pharmacist before using it. FDA's advice for compounded GLP-1 injectables is not to use product that arrives warm [2]. The label does not say why tirzepatide must be kept cool; it sets the limits.
Freezing
Do not freeze tirzepatide, and do not use it if it has been frozen, even after it thaws [1].
Traveling with tirzepatide
The same label limits apply on the road. Count travel days toward the 21-day or 30-day room-temperature total, keep the carton out of light and no warmer than 86°F, and do not let it freeze [1]. A weekly dose that falls during a trip can be moved to a different day if doses stay at least 72 hours apart [3]. Our sources do not cover airline or airport security rules; check with your airline and the TSA before you fly. Timing tips are on how to use tirzepatide.
Compounded tirzepatide vials and powder
Compounded tirzepatide is not FDA-approved, and FDA does not review compounded drugs for safety, effectiveness or quality [2]. The Zepbound storage rules do not apply to it. Storage and discard dates depend on the pharmacy that made it, so follow the pharmacy's label exactly. FDA's general advice for compounded multi-dose GLP-1 vials is to discard them 28 days after first use, use a new sterile needle and syringe for each dose, and not use injectables that arrive warm [2].
We do not give mixing or storage instructions for tirzepatide powder. Powders sold as "research use only" are not for people; FDA has warned sellers of tirzepatide labeled that way [2]. See research-use-only GLP-1s and compounded tirzepatide.
How long tirzepatide lasts in your body
Shelf life and body clearance are different questions. Once injected, tirzepatide has an elimination half-life of about 5 to 6 days [4], which is why it is dosed weekly. More on timing in tirzepatide results.

Questions people ask
Does tirzepatide need to be refrigerated?
Brand Zepbound is stored at 36°F to 46°F (2°C to 8°C). Single-dose pens and vials can be kept up to 86°F (30°C) for a total of 21 days if needed [1]. Compounded vials follow the pharmacy's label.
How long can tirzepatide be out of the fridge?
Single-dose pens and vials: up to 21 days total at or under 86°F (30°C). KwikPens and multi-dose vials: up to 30 days at room temperature, or 30 days after first use, or 4 doses, whichever comes first [1].
How long does tirzepatide last in the fridge once opened?
A Zepbound KwikPen or multi-dose vial must be thrown away 30 days after first use or after 4 weekly doses, even if medicine remains [1].
Can you freeze tirzepatide?
No. The label says do not freeze it and do not use it if it has been frozen [1].
What happens if tirzepatide gets warm?
Time up to 86°F (30°C) counts toward the label's room-temperature limit of 21 days for single-dose forms and 30 days for multi-dose forms [1]. The label gives no allowance for temperatures over 86°F, so ask your pharmacist before using product that got hotter.
Does compounded tirzepatide need to be refrigerated?
Follow the pharmacy's label, because storage depends on how that pharmacy made it. FDA advises discarding compounded multi-dose GLP-1 vials 28 days after first use and not using injectables that arrive warm [2].
Can you use tirzepatide after the expiration date?
The Zepbound storage instructions give no guidance for use past the expiration date [1]. Ask your pharmacist for a replacement rather than injecting expired product.
How do you travel with tirzepatide?
Apply the label limits: keep it in the carton, at or under 86°F (30°C), unfrozen, and count travel days toward the 21-day (single-dose) or 30-day (multi-dose) room-temperature total [1].
How long does tirzepatide last in your body?
Its elimination half-life is about 5 to 6 days, so levels fall by about half every 5 to 6 days after a dose [4].
