The cheapest tirzepatide is the lowest total yearly cost for a legally prescribed tirzepatide product, counting the drug, required memberships or visits, and the price at your maintenance dose.
- Cheapest brand
- Zepbound $299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg) a month on LillyDirect [2]
- Refill rule
- $449 only if you refill 7.5 mg and up within 45 days; otherwise $499 or $699 [2]
- Lowest compounded we list
- $117 a month on a prepaid 6- or 12-month plan, plus membership [1]
- Medicare Bridge
- $50 a month for Zepbound KwikPen if you qualify, through Dec 31, 2027 [5]
The short answer
For an FDA-approved drug, LillyDirect is the floor. TrumpRx lists Zepbound "from $299" but its Zepbound page sends buyers to LillyDirect, so it is the same price, not a lower one [4]. Online clinics that sell brand Zepbound, such as Ro, charge the same LillyDirect drug prices plus a membership [6].
Compounded tirzepatide costs less on paper. It is not FDA-approved, FDA does not review it for safety, effectiveness or quality, and FDA has received more than 730 adverse event reports for it [7]. The legal room for it is narrow (see compounded tirzepatide). We list only compounded sellers that did not meet our avoid rule.

First-year cost, with the arithmetic
Monthly prices hide the two things that drive a year's cost: how many fills you buy and which dose you end up on. LillyDirect counts a month as 28 days [2], so a year is 13 fills. The Zepbound label starts at 2.5 mg for 4 weeks, then 5 mg, then allows 2.5 mg steps no sooner than every 4 weeks; maintenance doses are 5, 10 or 15 mg [15]. Your prescriber sets your actual schedule. The examples below use the label's fastest pace only to show the math.
| Scenario | Arithmetic | Year total |
|---|---|---|
| Zepbound, stay on 5 mg | 1 x $299 + 12 x $399 = $299 + $4,788 | $5,087 |
| Zepbound, step up to 10 mg | $299 (2.5 mg) + $399 (5 mg) + 11 x $449 (7.5 mg, then 10 mg) = $698 + $4,939 | $5,637 |
| Same, with one 10 mg refill after day 45 | $5,637 + ($699 - $449) | $5,887 |
| Zepbound through Ro, step up to 10 mg | $5,637 + 12 x $74 annual-plan membership = $5,637 + $888 | $6,525 |
| Zepbound via Medicare Bridge | 12 x $50 | $600 |
| IVIM compounded, 12 months | 12 x $117 + 11 x $75 membership (first month free) = $1,404 + $825 | $2,229 (if both fees apply; the page is unclear) |
| bmiMD compounded at its lowest advertised rate | 12 x $179 | $2,148 (if that rate applies all year) |
| Maximus compounded | 12 x $249.99 | $2,999.88 |
| Found compounded, 12-month plan | 13 x $239 | $3,107 plus labs |
| OrderlyMeds compounded, monthly after intro | $299 (first 2 months) + 10 x $299 = $299 + $2,990 | $3,289 |
| Found compounded, 1-month plan | $219 + 12 x $359 = $219 + $4,308 | $4,527 plus labs |
| Direct Meds compounded | 12 x $399 | $4,788 |
Two things stand out. First, the cheapest brand year still costs about twice the cheapest compounded year. Second, the cheapest compounded sticker prices come with prepaid terms: IVIM bills 6- or 12-month programs and charges $100 for each unshipped 3-month portion if you cancel [16].
Hidden costs that change the total
- Needles and syringes. The Zepbound vial needs a syringe and needle and the KwikPen needs pen needles, sold separately [2]. The label says to use a new syringe and needle for each injection [15].
- Visit or membership fees. LillyDirect sells the drug, not the visit. Ro charges $74 to $149 a month [6]; LifeMD charges a $149 monthly program fee after a $39 first month [17]; PlushCare charges $129 for a first visit without insurance [18].
- Consult fees. Henry Meds charges $30 for each evaluation that ends in a prescription [19].
- Labs. Found excludes labs from its plan prices [12].
- Intro prices that end. OrderlyMeds doubles from $149 to $299 a month after the 2-month intro [9].
- Dose step-ups. LillyDirect charges more for higher doses up to 7.5 mg [2]. FancyMeds says higher compounded doses cost more than its $399 4.8 mg price [20].
- Lock-ins and 28-day billing. Found bills every 28 days, which is 13 charges a year [12]. Henry Meds may bill the remaining balance of a multi-month plan if you cancel [19].
- Late refills. Missing the 45-day window on LillyDirect raises a 10 to 15 mg month from $449 to $699 [2].
Ways to pay less, legally
- Ask your own doctor to send the prescription to LillyDirect. That removes the telehealth membership from the math [2].
- Refill within 45 days so 7.5 mg and above stays at $449 [2].
- Pick the KwikPen or the vial on price alone: LillyDirect charges the same for both [2].
- If your employer plan covers Zepbound, the savings card can cut a fill to $25 through Dec 31, 2026 [8].
- If you are on Medicare Part D and meet the BMI criteria, the GLP-1 Bridge charges a $50 copay for the Zepbound KwikPen; vials are excluded [21] [5].
- Use HSA or FSA dollars where the program allows it. Form Health says its program is HSA/FSA eligible [22].
- Consider whether a cheaper approved drug fits. Foundayo pills cost $149 to $299 a month on LillyDirect, with the lower 2.5 and 5.5 mg prices temporary through Dec 31, 2026 [23]. See GLP-1 cost comparison and Foundayo.

Cheap prices we do not recommend
Some of the lowest advertised prices come from programs we rate avoid, such as Mochi Health ("as low as $90" for compounded tirzepatide, plus membership) [27] and TrimRx ($299 to start) [28]. The reasons are below.
- MedVi: BBB rating F with 751 complaints, 135 unanswered [29]; FDA warning letter dated Feb 20, 2026 [30]. Read our MedVi review
- TrimRx: BBB rating F and a BBB "pattern of complaints" alert: 2,598 complaints in 3 years about billing and cancellations [31]. Read our TrimRx review
- Fridays: BBB rating F; 65 complaints in the last 12 months, 63 unanswered, mostly billing and refunds [32]. Read our Fridays review

The cheapest vial online is often not legal
"Research use only" tirzepatide sold without a prescription is not a cheaper version of anything. FDA says sellers of these products ship them with dosing instructions, and their quality is unknown and may be harmful [7]. Read what research-use-only GLP-1s are.
Insurance and Medicare
Insurance changes the ranking. With a commercial plan that covers Zepbound, $25 a fill beats every cash price, though the card caps savings at $100 a month and $1,300 a year [8]. If your plan does not cover it, the savings card on the KwikPen matches LillyDirect at $299 to $449 [8]. Medicare members cannot use the savings card but may qualify for the $50 Bridge copay [5].
