How we check

How GLP-1 medicines work and how long they take

GLP-1 stands for glucagon-like peptide-1, a hormone your body makes; drugs such as Wegovy are "glucagon-like peptide-1 (GLP-1) receptor agonists" that act on the same receptor [1][2]. They lower calorie intake, likely through appetite, and slow stomach emptying [3]. Weekly shots reach steady blood levels after about 4 to 5 doses [3][2], but climbing to the top dose takes about 4 to 5 months, and trials measured weight loss at 68 to 72 weeks [4][5].

Zepbound 15 mg weekly; Wegovy 7.2 mg weekly; Foundayo 17.2 mg daily [6][1][7]

What GLP-1 is

"GLP-1 is a physiological hormone that has multiple actions on glucose, mediated by the GLP-1 receptors" [2]. The gut releases it after you eat. A GLP-1 medicine is a drug that switches on that receptor. People say "GLP-1" or "GLPs" for the whole drug class, which today includes:

How GLP-1 drugs work for weight loss

  • Less hunger, fewer calories. For tirzepatide, the label says it "decreases calorie intake" and the effect is "likely mediated by affecting appetite." The GIP and GLP-1 receptors sit in brain areas that control appetite [3].
  • Slower stomach. Semaglutide delays early stomach emptying after meals [2]. With tirzepatide, the delay is largest after the first dose and fades over time [3].
  • Blood sugar. Semaglutide lowers blood sugar by stimulating insulin release [2].
  • Mostly fat, some muscle. In a SURMOUNT-1 substudy, tirzepatide cut fat mass 33.9% and lean mass 10.9% at 72 weeks [8]. See how to keep muscle on a GLP-1.

Do weight loss shots speed up metabolism or burn fat directly? The labels we read describe lower calorie intake and slower stomach emptying, not a metabolism boost [3][2].

How long does it take for a GLP-1 to work?

There are three clocks: how fast the drug reaches your blood, how long the dose climb takes, and how long weight loss takes to build.

GLP-1 timeline, from the FDA labels and trials
WhenWhat happensSource
Day 1 to 3Peak blood levels: about 24 hours after a tirzepatide shot (range 8 to 72 hours); 1 to 3 days after semaglutide[3][2]
After the first doseStomach emptying slows most after the first tirzepatide dose[3]
Weeks 4 to 5Steady blood levels: 4 weeks for tirzepatide, 4 to 5 weeks for semaglutide[3][2]
Weeks 1 to 16Wegovy steps up every 4 weeks: 0.25, 0.5, 1, 1.7 mg; maintenance from week 17[1]
Weeks 1 to 20+Zepbound starts at 2.5 mg and rises 2.5 mg no faster than every 4 weeks, so 15 mg comes at week 21 at the earliest[6]
Days 1 to 90+Wegovy pill: 1.5, 4 and 9 mg for 30 days each, then 25 mg from day 91[1]
68 to 72 weeksWhere the main trials measured results: 14.9% average loss on semaglutide 2.4 mg, 20.9% on tirzepatide 15 mg[4][5]
Averages, not promises. If a dose step is hard to tolerate, the Wegovy label allows delaying the next step by 4 weeks [1].

When does a GLP-1 kick in? Blood levels peak within days, and the tirzepatide label says the stomach-slowing effect is largest after the first dose [3]. Weight change builds over months, and how fast varies from person to person. Drug-specific detail: tirzepatide results, semaglutide results, Wegovy results.

Does a GLP-1 pill work as well as the shot?

There is no head-to-head answer across brands. The Wegovy label says side effects of the 25 mg pill are similar in type and frequency to the 2.4 mg shot [1]. Foundayo's label trial in adults without diabetes reported 11.1% average loss on 17.2 mg at 72 weeks, vs 2.1% on placebo [7]; SURMOUNT-1 reported 20.9% on tirzepatide 15 mg [5]. Different trials, different people, so do not compare them directly. Pills have their own rules: the Wegovy pill is taken in the morning on an empty stomach with up to 4 ounces of water [1]; Foundayo with or without food [7]. Compare: Foundayo vs Wegovy pill and Wegovy pill vs injection.

What is the highest dose of GLP-1 for weight loss?

  • Zepbound (tirzepatide): 15 mg once weekly [6].
  • Wegovy shot (semaglutide): 7.2 mg once weekly for adults who tolerated 2.4 mg for at least 4 weeks and need more weight loss [1].
  • Wegovy pill: 25 mg once daily [1].
  • Foundayo (orforglipron): 17.2 mg once daily; 9 mg maximum with strong CYP3A4 inhibitors [7].

How much GLP-1 should you take? Only what your prescriber orders under the product's label. Compounded vials are measured in units that depend on the concentration printed on the pharmacy label; follow your prescriber and the pharmacy label, and ask the pharmacist. Dosing guides: tirzepatide, semaglutide.

When to take a GLP-1 shot

Weekly shots can be taken any time of day. Tirzepatide can be taken with or without meals [9], and Wegovy's time of day can change without a dose change [1]. Pick a day you will remember. Missed-dose windows differ by brand: 4 days for Zepbound and Mounjaro, 5 days for Ozempic, and for the Wegovy shot, take it only if the next dose is more than 48 hours away [6][2][1].

Are GLP-1 drugs safe?

They are FDA-approved with known risks. The most common side effects are nausea, diarrhea, vomiting and constipation [3][1]. In weight trials, 6.8% stopped Wegovy 2.4 mg for side effects (3.2% placebo) [1], and 4.8% to 6.7% stopped Zepbound (3.4% placebo) [3]. Labels warn about pancreatitis, gallbladder disease, kidney injury from fluid loss, allergic reactions and aspiration during anesthesia, and carry a boxed warning for thyroid C-cell tumors in rodents [3][1].

Can a GLP-1 cause low blood sugar? Rarely on its own: in Zepbound's trial of adults without diabetes, 0.3% had blood sugar under 54 mg/dL vs 0% on placebo, while the risk rose with a sulfonylurea in people with diabetes [3]. Type 1 diabetes is outside the labels we review here; ask your diabetes clinician. Compounded and research-use products carry extra risk because FDA does not review them [10]. Safety signals to act on

Severe stomach pain that will not go away can mean pancreatitis; stop the drug and call your clinician [3]. Swelling of the face or throat or trouble breathing needs emergency care. Do not use a GLP-1 with a personal or family history of medullary thyroid carcinoma or MEN 2 [3]. Zepbound and Wegovy are stopped when pregnancy is recognized [3][1]. Call 911 in an emergency.

Should you take a GLP-1?

That is a decision for you and a licensed clinician. The weight labels cover adults with obesity, or overweight with a weight-related condition [6][7]. Questions worth asking: Which drug fits my health conditions? What will it cost at my maintenance dose? What happens if I stop? In trials, people who stopped regained much of the weight [11][12]. Questions people ask

What does GLP stand for?

GLP-1 stands for glucagon-like peptide-1. The Wegovy label describes the drug as a "glucagon-like peptide-1 (GLP-1) receptor agonist" [1], and the Ozempic label calls GLP-1 "a physiological hormone" [2].

How do GLP-1 drugs work?

They activate the GLP-1 receptor (tirzepatide also activates GIP). This lowers calorie intake, likely through appetite, and slows stomach emptying [3]; semaglutide also lowers blood sugar by stimulating insulin release [2].

How long does it take for a GLP-1 to work?

Blood levels peak within 1 to 3 days of a weekly shot and become steady after 4 to 5 weeks [3][2]. Doses can climb for 4 to 5 months, and the main trials measured average weight loss at 68 to 72 weeks [4][5].

How fast do you lose weight on a GLP-1?

It varies. In STEP 1, average loss was 14.9% at 68 weeks on semaglutide 2.4 mg [4]; in SURMOUNT-1, 20.9% at 72 weeks on tirzepatide 15 mg [5]. These are group averages.

Are GLP-1s safe?

They are FDA-approved with known risks. Common side effects are stomach-related, and labels warn of pancreatitis, gallbladder disease and a boxed warning for thyroid C-cell tumors in rodents. About 6.8% stopped Wegovy 2.4 mg for side effects vs 3.2% on placebo [1].

Can a GLP-1 cause low blood sugar?

Rarely on its own. In adults without diabetes on Zepbound, 0.3% had blood sugar under 54 mg/dL vs 0% on placebo; risk rises with insulin or a sulfonylurea [3].

What is the highest dose of GLP-1 for weight loss?

Zepbound 15 mg weekly [6], Wegovy 7.2 mg weekly or 25 mg daily as a pill [1], and Foundayo 17.2 mg daily [7].

Does a GLP-1 pill work as well as the shot?

No trial has compared all of them head to head. The Wegovy label says the 25 mg pill's side effects are similar to the 2.4 mg shot [1]. Foundayo's trial reported 11.1% average loss on 17.2 mg at 72 weeks [7], a different study from the shot trials.
