Wegovy pill vs injection: results, cost and side effects

Is Wegovy a pill or an injection?

Both. The Wegovy label covers a weekly injection and, since Dec 22, 2025, a daily tablet ^{[4] [1]}. Both contain semaglutide. The tablet is approved for adults; the injection is also approved for teens 12 and older and for MASH, a liver condition ^[4].

Does the pill work as well as the shot?

Close, in separate trials. OASIS 4 compared the 25 mg pill with placebo over 64 weeks ^[1]. Counting everyone regardless of whether they kept taking it, the pill group lost 13.6% versus about 2% on placebo; in Novo's estimate if everyone had stayed on treatment, 16.6% versus 2.7% ^[1]. In STEP 1, the 2.4 mg injection produced 14.9% versus 2.4% at 68 weeks ^[2]. The trials differ in length and population, so the numbers are not a head-to-head result.

The injection has one option the pill lacks: a 7.2 mg dose for adults who tolerate 2.4 mg for at least 4 weeks and need more weight loss ^[4]. See Wegovy results.

Side effects: same or different?

Mostly the same. The label says the 25 mg tablet's side effects were similar in type and frequency to the 2.4 mg injection ^[4]. For the injection, that means nausea in 44%, diarrhea in 30%, and vomiting and constipation in 24% each, versus 16%, 16%, 6% and 11% on placebo ^[4]. The tablet added dysesthesia, an altered skin sensation, in 5% versus 0% ^[4].

Two tablet-only issues matter. The pill raised levothyroxine levels by 33%, so thyroid medicine may need monitoring ^[4]. And the tablet contains an absorption enhancer (SNAC) that shows up in breast milk, which is why the label advises against breastfeeding on it ^[4]. Neither form is shown to be safer overall. Is the pill cheaper than the shot?

Yes, on self-pay. At maintenance the pill is $299 a month and the 2.4 mg pen $349 ^[3]. Over a year on the label schedule:

• Pill, 12 bottles of 30: $149 + $199 + $299 for the first three months, then 9 x $299 = $647 + $2,691 = $3,338 ^[3].
• Pen, 13 boxes of 4 weekly pens: 2 x $199 intro fills, then 11 x $349 = $398 + $3,839 = $4,237 ^[3].

The $199 pen intro covers only the first two fills of 0.25 or 0.5 mg for new self-pay patients and runs through Dec 31, 2026 ^[3]. With commercial coverage, either form can be as little as $25 a month ^[3]. Medicare's GLP-1 Bridge covers both at $50 a month for eligible members ^[7]. Switching between the pill and the shot

This is the one GLP-1 switch the label spells out ^[4]:

• From the 2.4 mg injection to tablets: start 25 mg tablets one week after the last injection.
• From 25 mg tablets to the injection: start 2.4 mg weekly the day after the last tablet.

Is Rybelsus the same as the Wegovy pill?

Rybelsus is another Novo Nordisk semaglutide tablet ^[8]. We did not review its label for this page. The Wegovy label says not to take Wegovy with other semaglutide-containing products ^[4].
