Switching GLP-1s: Wegovy, Zepbound, Ozempic and Mounjaro

What the labels say, drug by drug

Every GLP-1 label tells a prescriber how to start a new patient. In the label sections we reviewed, none describes how to move a patient over from a different GLP-1 drug. Here is each label's own starting point:

The rule the weight-loss labels share: one GLP-1 at a time

Zepbound's label says it is not recommended with other tirzepatide products or any GLP-1 receptor agonist ^[1]. Wegovy's label says the same for other semaglutide products and any other GLP-1 receptor agonist ^[2]. Foundayo's label says it is not recommended with another GLP-1 receptor agonist ^[3]. So for "can I take Ozempic and Mounjaro in the same week?", the tirzepatide and semaglutide labels we reviewed point the same way: no.

The one switch the label spells out: Wegovy shot and pill

• Injection to tablets: start 25 mg tablets one week after the last 2.4 mg injection ^[2].
• Tablets to injection: start the 2.4 mg weekly injection the day after the last 25 mg tablet ^[2].

Common switches and what applies

Semaglutide to tirzepatide (Wegovy or Ozempic to Zepbound or Mounjaro)

The usual reason is weight. In SURMOUNT-5, tirzepatide averaged 20.2% weight loss at 72 weeks and semaglutide 13.7% ^[4]. That trial compared people who started on each drug; it did not test switching. The Zepbound label start is 2.5 mg weekly ^[1]. How soon after your last semaglutide dose to start, and at what dose, is your prescriber's call; the labels do not specify a waiting period. Tirzepatide may make birth control pills less effective for 4 weeks after starting and after each dose increase, so the label advises a non-oral or backup method ^[5].

Tirzepatide to semaglutide (Zepbound to Wegovy)

Reasons include coverage, cost or side effects. Wegovy's label start is 0.25 mg weekly ^[2]. Medicare's GLP-1 Bridge, for example, covers Wegovy pens and tablets but only the Zepbound KwikPen ^[7]. Semaglutide has a long half-life, and the Wegovy label says to stop it at least 2 months before a planned pregnancy ^[2].

Ozempic to Wegovy, or Mounjaro to Zepbound

These are the same active ingredients under different labels: semaglutide for Ozempic and Wegovy, tirzepatide for Mounjaro and Zepbound ^{[6] [2] [5] [1]}. People usually switch because the diagnosis or insurance changed. The strengths overlap, and the prescriber decides the dose. See Ozempic vs Wegovy and Mounjaro vs Zepbound.

Saxenda to Wegovy or Mounjaro

We did not review the Saxenda label for this page, and the Wegovy and Mounjaro labels give no conversion from it. Their own starting doses apply as listed in the table, unless your prescriber decides otherwise ^{[2] [5]}.

Gaps, restarts and weight regain

Gaps between drugs matter. The Wegovy label says that if 2 or more weekly doses in a row are missed, the provider may restart the dose escalation at a lower dose ^[2]. Foundayo's label restarts escalation after 7 or more missed daily doses ^[3]. Zepbound and Mounjaro allow a missed dose within 4 days ^[1].

Stopping without a replacement can undo progress. In SURMOUNT-4, people switched from tirzepatide to placebo regained 14.0% of body weight over a year, while those who stayed on tirzepatide lost another 5.5% ^[8]. In the STEP 1 extension, people regained about two-thirds of the weight lost within a year of stopping semaglutide ^[9]. These are stopping studies, not switching studies, so they cannot say whether a switch causes weight gain. See stopping tirzepatide and stopping semaglutide.
