How we check

What happens when you stop Semaglutide

The labels set no end date: Wegovy is approved to reduce excess weight and keep it off long term [1]. When people stop, weight tends to return. In the STEP 1 extension, people regained 11.6 of the 17.3 percentage points they had lost within a year of stopping semaglutide 2.4 mg [2].

Medical review: pending. A licensed clinician has not yet reviewed this page.

Stopping semaglutide means ending a weekly injection or daily tablet, after which the drug leaves the body slowly and appetite and weight effects fade.

Stop at least 2 months ahead [1]

How long you can stay on semaglutide

There is no labeled maximum. Wegovy is approved to reduce excess body weight and maintain that reduction long term [1]. In the SELECT heart trial, people took Wegovy injection for a median 37.3 months [1]. Longer-term safety is still being studied; the boxed thyroid tumor warning applies for as long as you use it [1].

Many people ask whether they must take it forever. The label does not say you must. What the data show is what happens after stopping, covered in the next section. Costs over time are on our semaglutide cost page.

What happens after you stop

The STEP 1 extension followed people for a year after semaglutide 2.4 mg and placebo were stopped at week 68. The semaglutide group had lost 17.3%. By week 120 they had regained 11.6 percentage points, about two-thirds, leaving a net loss of 5.6% vs 0.1% for placebo [2]. Improvements in heart and metabolic measures largely returned toward where they started [2].

Semaglutide also leaves the body slowly. The label tells people planning a pregnancy to stop at least 2 months ahead because of its long half-life [1]. Side effects tied to the drug would be expected to fade as it clears, but the label does not give a timeline.

Cold turkey, tapering and restarting

The labels describe how to raise the dose, not how to lower it, and contain no taper schedule. We did not find a withdrawal syndrome in the label sections we reviewed. Talk to your prescriber before stopping, because the label flags insulin and sulfonylurea doses for adjustment alongside semaglutide [1].

Restarting is where the label is specific: if 2 or more Wegovy doses in a row are missed, contact your provider, who may restart at a lower dose [1]. Stopping for 3 months and then resuming at your old dose is not a label schedule.

Stopping before surgery or pregnancy

Surgery. The label says there is not enough data to recommend pausing semaglutide or changing fasting before anesthesia, but warns of a risk of food entering the lungs; tell your surgeon and anesthesia team you take it [1]. They decide whether to hold doses. Details in semaglutide interactions.

Pregnancy. Stop at least 2 months before a planned pregnancy, and stop Wegovy when a pregnancy is recognized [1].

Side effects that make people quit

In the Wegovy trials, 6.8% of adults on 2.4 mg stopped because of side effects vs 3.2% on placebo, mostly nausea, vomiting and diarrhea [1]. Before quitting, ask your prescriber about the label option of delaying a dose increase by 4 weeks [1].

  • Heartburn, burping and bloating: reflux, burping (eructation) and bloating are listed reactions [1]. The label does not mention sulfur burps by name.
  • Diarrhea: 30% vs 16% on placebo [1]. Diarrhea that will not stop can cause dehydration and kidney injury; call your prescriber [1].
  • Vomiting you cannot stop: call your prescriber the same day; severe vomiting can lead to kidney injury [1].
  • Hair loss: 3.3% vs 1% on placebo [1]. Whether hair regrows after stopping is not addressed in the label.

When it seems to stop working

The label does not describe semaglutide losing effect over time, and our sources do not measure it. Response varies from the start: in STEP 1, 13.6% of people on semaglutide did not reach 5% weight loss at 68 weeks [3]. If your weight has stalled, your prescriber can review your dose, which for Wegovy can go to 7.2 mg in some adults [1], or a switch; see switching GLP-1 medicines and semaglutide results.

Stopping is not always the fix

Severe stomach pain that does not go away can be pancreatitis: stop the medicine and call your prescriber right away [4]. If you use insulin or a sulfonylurea, your doses may need changing when you stop or start semaglutide [1]. Call 911 in an emergency.

Questions people ask

How long can you take semaglutide?

No label limit. Wegovy is approved for long-term weight maintenance, and SELECT trial participants took it for a median 37.3 months [1].

What happens when you stop taking semaglutide?

Weight tends to return. In the STEP 1 extension, people regained 11.6 of 17.3 percentage points lost within a year of stopping [2].

Can you stop semaglutide cold turkey?

The label has no taper schedule and we found no withdrawal syndrome in it. Talk to your prescriber first, especially if you take diabetes medicines [1].

Do you have to take semaglutide forever?

The label does not require it. Data show most of the lost weight returned within a year of stopping in the STEP 1 extension [2].

When should you stop semaglutide before surgery?

The label does not set a stop time; it says data are not enough to recommend pausing, and you should tell your providers you take it [1].

When should you stop semaglutide before pregnancy?

At least 2 months before a planned pregnancy, because of its long half-life [1].

Can I stop and restart semaglutide?

Ask your prescriber first. For Wegovy, missing 2 or more doses in a row may mean restarting at a lower dose [1].

Does semaglutide stop working after a while?

The label does not describe that, and our sources do not measure it. Response varies; 13.6% in STEP 1 did not reach 5% loss [3].

How long does semaglutide stay in your system after stopping?

The label gives no number of days. It advises stopping at least 2 months before a planned pregnancy because of the long half-life [1].
