How we check

Mounjaro cost with and without insurance

Mounjaro costs $499 to $1,112.16 a month without plan coverage, per Lilly's pages checked Oct 17, 2026: the list price is $1,112.16 for a 28-day supply of four pens [1], and Lilly's savings card brings it to $499 when a commercial plan does not cover it [2]. With a plan that covers it, the card can cut the cost to $25, but people with no insurance cannot use the card [2]. We could not confirm a LillyDirect self-pay price for Mounjaro from Lilly's own pages as of Oct 17, 2026.

Mounjaro is Eli Lilly's once-weekly tirzepatide injection approved to control blood sugar in people 10 and older with type 2 diabetes and to cut heart risk in high-risk adults with type 2 diabetes; it is not approved for weight loss [3].

What Mounjaro costs in each situation

Your price depends almost entirely on your insurance. Unlike Zepbound, Mounjaro has no LillyDirect self-pay price that we could confirm from Lilly; Lilly's Mounjaro pages we checked list only the list price and the savings card. Ask the pharmacy for its cash price before you fill. The GLP-1 Price Tracker shows how that compares with every other option.

Extra costs to expect

  • Dose increases. The label starts at 2.5 mg and adds 2.5 mg steps after at least 4 weeks if more blood sugar control is needed, up to 15 mg for adults [3].
  • Visits and labs for diabetes care, billed by your clinician.
  • Pen needles if you use the multi-dose KwikPen, which holds four doses [7]. The FDA approved a Mounjaro KwikPen in late January 2026, Fierce Pharma reported [8].
  • The card's end date. Mounjaro card savings run through Dec 31, 2026 [2].

How to get Mounjaro for $25

  1. Have type 2 diabetes. The card requires a prescription for an FDA-approved use [2], and Mounjaro's approved uses are for type 2 diabetes [3].
  2. Have commercial insurance. Medicare, Medicaid and other government plans are excluded [2].
  3. Confirm your plan covers Mounjaro. If it does, the card can lower the copay to $25 [2]. Our Zepbound and Mounjaro insurance guide shows how to check.
  4. Activate the card and show it at the pharmacy with each fill.

Mounjaro without diabetes: look at Zepbound

Mounjaro and Zepbound hold the same drug, tirzepatide, and the Zepbound label says not to use the two together [9]. Zepbound is the version approved for weight management and sleep apnea [7], and it has a cash price: $299 to $449 a month on LillyDirect when you refill within 45 days [4]. A prescription for Mounjaro without diabetes is off-label, so the Mounjaro savings card, which needs an approved use, will not apply [2]. See Mounjaro vs Zepbound: same drug, different label.

In a trial in the Zepbound label of adults with type 2 diabetes, people on 10 mg and 15 mg tirzepatide lost an average of 12.8% and 14.7% of body weight at 72 weeks, against 3.2% on placebo [12].

Medicare and Medicaid

For people with type 2 diabetes, Medicare says your Part D plan might cover GLP-1s [13]. Your cost then follows that plan's tiers. The Lilly savings card cannot be used with Medicare or Medicaid [2]. Medicaid rules differ by state; see Medicare and Medicaid GLP-1 coverage.

Mounjaro safety points

Mounjaro carries a boxed warning for thyroid C-cell tumors seen in rats; do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2 [12]. With basal insulin, 14% to 19% of trial patients had low blood sugar below 54 mg/dL, and with a sulfonylurea 9.9% to 13.8% [3]. Severe, lasting stomach pain (a possible sign of pancreatitis) and allergic reactions need prompt medical care. Oral birth control may work less well for 4 weeks after starting and after each dose increase [3]. For an emergency, call 911. See Mounjaro side effects.

Questions people ask

How much is Mounjaro without insurance?

Mounjaro's list price is $1,112.16 for a 28-day supply of four pens [1]. People without insurance cannot use Lilly's savings card [2], and we could not confirm a LillyDirect self-pay price for Mounjaro from Lilly as of Oct 17, 2026.

How do I get Mounjaro for $25?

You need commercial insurance that covers Mounjaro and a prescription for an approved use, which means type 2 diabetes. The Lilly savings card can then lower the cost to $25 for a 1-, 2- or 3-month fill, through Dec 31, 2026 [2].

What if my insurance does not cover Mounjaro?

With commercial insurance that does not cover it, the savings card can bring Mounjaro to as low as $499 for a 1-month fill, with savings capped at $647 a month [2].

Is there a generic for Mounjaro?

No generic Mounjaro is sold. Its ingredient is tirzepatide, also sold by Lilly as Zepbound for weight management [9]. Compounded tirzepatide is not a generic and is not FDA-approved [16].

Can I get Mounjaro without diabetes?

Mounjaro is approved only for type 2 diabetes uses [3], so a prescription without diabetes is off-label and the savings card requires an approved use [2]. Zepbound is the same drug approved for weight management, at $299 to $449 a month on LillyDirect [4].

Does Medicare cover Mounjaro?

For type 2 diabetes, Medicare says your Part D plan might cover GLP-1s [13]. The Medicare GLP-1 Bridge for weight management does not include Mounjaro [6].

How much does Mounjaro cost per month with the savings card?

As little as $25 if your commercial plan covers it, or as low as $499 if it does not. Savings are capped at $150 per covered month and $647 per uncovered month [2].
