How we check

Does insurance cover Ozempic? Diabetes, weight loss and your plan

Ozempic costs $25 to $499 a month depending on coverage, per NovoCare checked Oct 17, 2026: as little as $25 with Novo's savings offer when a commercial plan covers it, and $349 to $499 a month self-pay when it does not [2]. The diagnosis decides coverage: Ozempic is FDA-approved for type 2 diabetes, not weight loss, so a request without diabetes is off-label [1].

Ozempic insurance coverage means your health plan pays for Ozempic, a semaglutide injection approved to control blood sugar and lower heart and kidney risks in adults with type 2 diabetes [1].

Why the diagnosis decides Ozempic coverage

Ozempic's label covers three uses, all in adults with type 2 diabetes: blood sugar control, lowering the risk of heart attack, stroke and cardiovascular death in those with heart disease, and lowering kidney and heart risks in those with chronic kidney disease [1]. A plan reviewing a prior authorization request is checking for one of those. The same ingredient, semaglutide, is approved for weight management as Wegovy [4], so a request for weight loss belongs under Wegovy's rules instead. See Ozempic vs Wegovy.

Checking your plan

  1. Look up Ozempic on your plan's formulary in the member portal, or call the pharmacy line on your card.
  2. Note any prior authorization or step therapy flag. Ask exactly what the request must show, such as a type 2 diabetes diagnosis or recent lab results.
  3. Ask your prescriber's office to send the request with the matching chart notes.
  4. Once approved, activate Novo's savings offer if you have commercial insurance [2].

When the answer is no

  • Appeal if you have type 2 diabetes and the denial cites missing paperwork. Check the denial letter for the reason and any appeal deadline; we could not confirm a standard deadline, so ask your plan and act quickly.
  • Switch the request to Wegovy if your goal is weight management and your plan covers obesity drugs. See Wegovy insurance.
  • Pay cash for less: Ozempic $349 [2], or for weight loss the Wegovy pill from $149 [8] or Foundayo from $149 [9].
  • Avoid unapproved copies. FDA warns that counterfeit Ozempic has been found in the US [10].

Medicare and Medicaid

The Medicare GLP-1 Bridge does not cover Ozempic [3]. For type 2 diabetes, Medicare says your Part D plan might cover your GLP-1 [5]. Medicaid rules for weight use vary by state: in January 2026, 13 programs covered GLP-1s for obesity [11]. Some states that ended obesity coverage kept diabetes coverage, as South Carolina did [12]. MassHealth stopped covering anti-obesity drugs used only for weight loss on July 3, 2026 [13], and Medi-Cal ended adult weight-loss coverage on Jan 1, 2026 [14]. State by state: Medicare and Medicaid GLP-1 coverage.

Ozempic safety reminders

Ozempic has a boxed warning for thyroid C-cell tumors seen in rodents; do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2 [1] [4]. Low blood sugar is more likely if you also take insulin or a sulfonylurea. Seek care for stomach pain that is severe and will not ease, or for swelling, hives or trouble breathing. Stop at least 2 months before a planned pregnancy [1]. For an emergency, call 911.

Questions people ask

Does insurance cover Ozempic for weight loss?

Usually not as written, because Ozempic is approved only for type 2 diabetes uses, so weight-loss use is off-label [1]. Wegovy is the same ingredient approved for weight management; ask whether your plan covers Wegovy instead [4].

Does Aetna, UnitedHealthcare, Blue Cross or Kaiser cover Ozempic?

Coverage depends on your employer's plan, not the carrier name. The same insurer can run many plans with different drug lists. Steps: call the pharmacy number on your card, ask whether Ozempic is on your formulary and for which diagnosis, ask about prior authorization or step therapy, and ask your copay. If covered, Novo's savings offer can lower the cost to $25 a month [2].

How much is Ozempic with insurance?

If your commercial plan covers Ozempic, Novo's savings offer can bring it to as little as $25 a month by paying up to $100 of your copay. The offer is not for Medicare, Medicaid or other government plans [2].

Will insurance cover Ozempic for prediabetes, PCOS or fatty liver?

Ozempic is not approved for any of those [1]. On Medicare, prediabetes with a BMI of 27 or more qualifies for the GLP-1 Bridge, which covers Wegovy, Foundayo and the Zepbound KwikPen but not Ozempic [5] [3]. Wegovy injection is approved for MASH fatty liver disease with moderate to advanced fibrosis [4].

Does TRICARE, the VA or CHAMPVA cover Ozempic?

Coverage depends on the specific plan's formulary and rules, not its name. Ask the plan whether Ozempic is covered for your diagnosis. Novo's $25 offer excludes government plans [2].

Does MassHealth or Medi-Cal cover Ozempic?

For weight loss alone, no: MassHealth stopped covering anti-obesity drugs used solely for weight loss on July 3, 2026 [13], and Medi-Cal ended adult weight-loss GLP-1 coverage on Jan 1, 2026, while still covering other uses when medically necessary [14].

Does GoodRx cover Ozempic?

GoodRx is not insurance. Novo listed GoodRx among its self-pay partners [16]. NovoCare's own self-pay price is $349 a month for 0.25 to 1 mg [2].

How do I know if my insurance covers Ozempic?

Check your plan's formulary online or call the pharmacy number on your card, and ask about prior authorization and step therapy. Ozempic's approved use is type 2 diabetes, so make sure that diagnosis is on the request [1].
