A Mounjaro diet is the eating and drinking pattern a person follows while taking Mounjaro (tirzepatide) for type 2 diabetes; the label sets no specific diet.
- Active ingredient
- Tirzepatide (GIP and GLP-1 receptor agonist) [1]
- With food?
- With or without meals, any time of day [1]
- Fasting glucose drop
- 55 to 60 mg/dL at 40 weeks (SURPASS-2) [1]
- Low blood sugar alone
- 0% on all doses vs 1% placebo [1]
- Pill absorption
- Acetaminophen peak fell 50% after first 5 mg dose [1]
How Mounjaro lowers blood sugar
Tirzepatide boosts insulin release and lowers glucagon when blood sugar is high, and slows stomach emptying [1]. In SURPASS-2, fasting glucose fell by 55 to 60 mg/dL on Mounjaro over 40 weeks, and A1C fell by 2.0 to 2.3 points [1]. In SURPASS-1, A1C fell 1.7 to 1.8 points vs 0.1 on placebo [1]. The label measures these changes over months, not after a single dose.
Low blood sugar and how to avoid it
| Setting | Placebo | Mounjaro 5 / 10 / 15 mg |
|---|---|---|
| Mounjaro alone | 1% | 0% / 0% / 0% |
| With basal insulin | 13% | 16% / 19% / 14% |
| With a sulfonylurea | Not listed | 13.8% / 9.9% / 12.8% |
The risk comes mainly from insulin and sulfonylureas, and prescribers may lower those doses when Mounjaro starts [1]. In Zepbound's trial of people without diabetes, low blood sugar under 54 mg/dL occurred in 0.3% vs 0% on placebo [2]. If you use insulin or a sulfonylurea and eat much less than usual, ask your care team how to adjust and how to treat a low.
Alcohol on Mounjaro
Alcohol, beer and wine do not come up in the parts of the Mounjaro label we read [1]. Raise it with your prescriber, who will weigh your other medicines (especially insulin or a sulfonylurea), any history of pancreatitis, and how your stomach is handling the drug. If pancreatitis is suspected, the label says to stop Mounjaro [1].
What to eat and what to limit
No foods are banned on the label, and the dose can be taken before or after eating [1]. Because tirzepatide slows stomach emptying, nausea (12% to 18%) and decreased appetite (5% to 11%) are common [1]. Smaller meals and stopping when full are practical steps many people use; a dietitian can help with a plan that fits your diabetes. Eggs, potatoes, ham or diet soda are not addressed in the label. Our GLP-1 diet plan covers meal ideas.
The label sets no calorie or protein target. Tirzepatide causes more fat loss than lean mass loss [2], yet about 25% of weight lost in a SURMOUNT-1 substudy was lean mass [3]. Ask your care team about protein and strength training; see keeping muscle on a GLP-1. Zepbound, the weight-management version, is labeled for use with a reduced-calorie diet and more physical activity [4].
Staying hydrated and timing your pills
Kidney injury, sometimes needing dialysis, has been reported in people on Mounjaro who became dehydrated after vomiting or diarrhea [1]. Keep up your fluids, and call your prescriber if you cannot hold them down. Tirzepatide also slows how quickly tablets are absorbed: a first 5 mg dose cut acetaminophen's peak level by 50% [1]. No vitamin is named as a problem. Ask your pharmacist about spacing vitamins and other medicines, and see Mounjaro interactions.
Mounjaro is approved for type 2 diabetes, not weight loss [1]. Trial averages are on Mounjaro results, what happens to appetite and weight after the last dose is on stopping Mounjaro, the semaglutide version of this page is what to eat on Ozempic, and prices are on Mounjaro cost.
Questions people ask
Does Mounjaro have vitamin B12?
Mounjaro's active ingredient is tirzepatide, a GIP and GLP-1 receptor agonist [1]; vitamin B12 is not its active ingredient. Ask your clinician to check your B12 level if you are concerned.
Does Mounjaro lower blood sugar?
Yes. In SURPASS-2, fasting glucose fell 55 to 60 mg/dL and A1C fell 2.0 to 2.3 points at 40 weeks [1].
Can Mounjaro cause low blood sugar?
Rarely on its own: 0% vs 1% on placebo. With basal insulin it was 14% to 19%, and with a sulfonylurea 9.9% to 13.8% [1].
Can Mounjaro cause low blood sugar if you are not diabetic?
In Zepbound's trial of people without diabetes, lows under 54 mg/dL occurred in 0.3% vs 0% on placebo [2].
Can you drink alcohol on Mounjaro?
The Mounjaro label sections we read say nothing about alcohol [1]. Ask your prescriber, especially if you take insulin or a sulfonylurea.
Should Mounjaro be taken with food?
It can be taken with or without meals, at any time of day [1].
What vitamins should not be taken with Mounjaro?
The label names none, but Mounjaro slows absorption of oral medicines; acetaminophen's peak level fell 50% after a first 5 mg dose [1]. Ask your pharmacist.
How much water should I drink on Mounjaro?
The label gives no amount but warns that dehydration from vomiting or diarrhea has caused kidney injury [1].
Can you lose weight on Mounjaro without exercise?
Mounjaro is a diabetes medicine. Zepbound, the same medicine for weight, is labeled for use with a reduced-calorie diet and more physical activity [4].
