How we check

Direct Meds review: cost, pharmacy and complaints

Direct Meds charges $297 a month for compounded semaglutide and $399 a month for compounded tirzepatide, with the doctor visit, supplies and shipping included [1]. FDA sent it a warning letter on Sept 9 2025 for claiming its compounded drugs used "the same active ingredient" as brand drugs [2]; those claims were not on its homepage or pricing page on Oct 17 2026, so we rate it Caution, provisionally.

Medical review: pending. A licensed clinician has not yet reviewed this page.

Direct Meds (directmeds.com, Inc. dba DirectMeds) is an online prescriber of compounded semaglutide and tirzepatide that also sells NAD, hormone and ED treatments.

Our verdict

Use caution

Flat, all-inclusive monthly prices, no contract and a large Trustpilot following. A Sept 2025 FDA warning letter for sameness claims keeps it at Caution; the cited claims were not found on its main pages when we checked. The rating is provisional: it has no confirmed BBB record, and we scanned only two pages.

FDA record
Warning letter dated Sept 9 2025 for misbranding: claims that its compounded semaglutide "contains the same active ingredient as branded semaglutide" [2]. Not found on the homepage or pricing page on Oct 17 2026.
BBB
No confirmed BBB record. We could not check Direct Meds's BBB profile as of Oct 17 2026, so this rating does not rest on a good BBB record and stays provisional until we do.
Best for
Cash buyers who want one monthly price for compounded semaglutide or tirzepatide with no contract.
Watch out
We have not yet confirmed this company's BBB record. We checked only the homepage and pricing page for the claims FDA cited, and Trustpilot reviewers report orders stuck pending and cold-chain concerns [3].

For context, brand drugs from the makers fall in a similar range, but you need your own prescriber: LillyDirect sells Zepbound at $299 to $449 a month by dose [4], and NovoCare sells the Wegovy pen at $349 a month for standard doses [5]. Our tirzepatide cost and semaglutide cost guides compare every channel.

Refunds and cancelling

Direct Meds says you get a full refund if a physician does not approve you, and you can cancel any time in the patient portal [1]. We did not read its full refund terms, so ask what happens to a shipped order before you buy.

What you get

You complete an online intake, a physician reviews it, and compounded semaglutide or tirzepatide ships from a US pharmacy that Direct Meds does not name [1]. The FDA letter lists the company in Bluffdale, Utah; the site now shows a Houston, Texas address [2] [6]. The states it serves were not listed on the pages we read.

Oral semaglutide drops and oral tirzepatide are referenced on the site [6] [1]. No compounded oral GLP-1 is FDA-approved; see our oral tirzepatide guide for what is.

Is Direct Meds legit? The FDA letter and the reviews

The FDA letter: On Sept 9 2025, FDA wrote to directmeds.com, Inc. dba DirectMeds about misbranding under sections 502(a) and 502(bb) of the drug law. It quoted three claims: "Compounded semaglutide contains the same active ingredient as branded semaglutide"; "Compounded formulations use the same active ingredient as the brand-name drug"; and "That's where buying compounded semaglutide online can make a big difference." FDA said its list was not complete [2]. FDA treats sameness claims as misleading because compounded drugs are not reviewed the way approved drugs are [7].

Status today: None of the three phrases appeared on the Direct Meds homepage or pricing page on Oct 17 2026 [6] [1]. We did not scan its blog or FAQ. A past letter with fixed claims is why we rate it Caution rather than Avoid. The same Sept 2025 wave reached Hims, Remedy Meds and others [8]; see our FDA warning letter tracker.

BBB: Not yet confirmed. A D or F rating, or a billing complaint pattern, would move Direct Meds to Avoid under our rating rules.

Trustpilot: 4.6 from 16,099 reviews, 11,563 of them in the last 12 months, on a profile claimed in May 2025. Negative reviews describe orders stuck in pending, concerns about shipments staying cold, missing supplies and trouble cancelling [3].

Who it fits

  • Fits: someone paying cash who wants one predictable monthly price for compounded semaglutide or tirzepatide and no contract.
  • Look elsewhere if: you want an FDA-approved drug, want to know the pharmacy before ordering, or want a cheaper compounded price. OrderlyMeds names its pharmacies, and bmiMD advertises lower "as low as" prices.

Know what you are taking

Compounded semaglutide and tirzepatide are not FDA-approved, and FDA does not review them for safety, effectiveness or quality [16]. FDA has warned of dosing errors with compounded injectable semaglutide, some leading to hospital care [9]. Follow your prescriber's instructions and the pharmacy label, and ask the pharmacist if anything is unclear. The Wegovy label's boxed warning covers thyroid C-cell tumors in rodents, and its warnings include pancreatitis: severe stomach pain that will not go away needs prompt care [10]. Call 911 for a severe allergic reaction.
