Household income and the fee

Orange County's median household income is $116,289 [5]. A $4,000 Sollis membership would be about 3.4% of that; a $2,250 MDVIP-style fee about 1.9%. The AAFP notes concierge practices usually serve higher-income patients and may bill insurance on top of the fee [7]. Insurance premiums and copays still apply in most concierge models.

Medicare members

With 18.5% of residents 65 or older [5], many Newport Beach and Irvine patients are on Medicare. Medicare does not cover concierge fees, and a doctor who accepts assignment cannot charge the fee for services Medicare covers [8]. MDVIP says its fee pays for an annual wellness program not usually covered by insurance, while patients keep Medicare for other care [9]. More in concierge medicine and Medicare.

Paying with tax-advantaged accounts

MDVIP says its fee may be reimbursed through an FSA after services are received but cannot be paid with an FSA card; that is MDVIP's position, not IRS guidance [10]. The new IRS rule for HSAs covers only direct primary care that does not bill insurance, with fees up to $150 a month per person [11].
