How we check

Concierge medicine cost: the Concierge Fee Index

Concierge medicine fees published on practice pages ran from $2,250 to $3,600 a year at MDVIP physicians and PartnerMD, and from $4,000 a year at Sollis Health, when we checked on October 17, 2026 [1][2][3][4]. You usually pay that on top of your insurance, because concierge practices keep billing insurance for covered care [5].

The Concierge Fee Index

Every row below is a published fee, with the date we read it and a link to where we read it. Ten rows come from the practice's own page; the Huntington Direct Primary Care row comes from its DPC Frontier directory listing, and the row says so. Rows are grouped by model and then by state. The order is not a ranking, and a listing is not an endorsement. For background on the model itself, start with our concierge medicine guide.

Published fees only, read on each practice's own page on October 17, 2026. MDVIP fees vary by physician, so the three MDVIP rows describe those doctors only [13].

How we build the Index

  • We include a fee only when a practice publishes it. We do not use prices from reviews, forums or sales calls.
  • We read the fee on the practice's own page and record the date we saw it. When a row comes from somewhere else, the table says so.
  • We list facts only: practice, city, model, fee, what is included and the link. We do not score, rank or recommend practices.
  • No one pays to be in the Index or to change a row. City guides can carry a labeled sponsored slot, which is separate. See how we make money.
  • Practices that do not publish a fee are listed in a separate table as not published, not left out.
  • Practices can send corrections through our contact page.

Hidden costs on top of the fee

  • Your insurance costs continue. Concierge practices keep billing insurance [5], so premiums, copays and deductibles still apply to covered visits. One Medical says copays or deductibles may apply to its scheduled visits [8].
  • Separate lab charges. One MDVIP physician page listed a $70 lab charge billed apart from the $2,930 fee [2].
  • Registration fees. KerixHealth charges $250 to register in Illinois and $100 per person (up to $300 a family) in Colorado and Virginia [7].
  • Labs and medicines. Plum Health lists cash-priced labs and medications separately from its membership [12].
  • Vaccines. Cleveland Clinic Concierge Medicine lists vaccinations at cost [14]. Ask whether that cost is billed on top of the fee.
  • Leaving early. PartnerMD asks for 30 days' notice to cancel and gives a prorated refund [3]. Ask every practice for its terms in writing.

Ways to pay less

  • Compare direct primary care. DPC fees in the Index ran $75 to $199 a month per adult, with no insurance billing for primary care visits [6][7]. See our direct primary care guide.
  • Check your employer. Employers now fund 60% of active DPC memberships tracked by Hint Health [21].
  • Ask about age tiers. PartnerMD has separate Adult (26+) and Young Adult (16 to 25) categories [3].
  • Consider a hybrid. One Medical's $199 yearly membership covers 24/7 messaging and video care, with visits billed to insurance [8].
  • Spread payments. MDVIP allows quarterly, semi-annual or annual payment [13]. One Arlington, Texas physician lists $600 a quarter or $1,200 every six months for a $2,400 fee [11]. The total stays the same.

HSA, FSA and taxes

Starting January 1, 2026, section 71308 of the One Big Beautiful Bill Act lets people with an HSA enroll in a direct primary care service arrangement and pay its fees from the HSA [10][23]. The arrangement qualifies only if fees total no more than $150 a month for one person, or $300 a month when it covers more than one person [10]. Rev. Proc. 2026-24 keeps those amounts at $150 and $300 for 2027 [24].

Concierge practices that bill insurance on top of a retainer do not qualify. Notice 2026-5 says an arrangement that bills separately for items or services through insurance or otherwise is not a direct primary care service arrangement [10]. The notice also says fees over the monthly cap can still be reimbursed as medical expenses but make the person ineligible to contribute to an HSA while enrolled, and that employer-paid DPC fees cannot be reimbursed from the HSA [10].

For concierge retainers themselves, we found no IRS guidance on whether the fee is a deductible medical expense or an HSA-eligible expense. MDVIP says its fee may be reimbursed through an FSA, MSA or similar account after the annual wellness services are received, and that it cannot be paid with an FSA debit or credit card. That is MDVIP's position, not IRS guidance [25]. Ask your plan administrator or a tax professional before you count on it.

Insurance and Medicare

Medicare doesn't cover membership fees for concierge care, and a doctor who accepts assignment can't fold extra charges for Medicare-covered services into the fee [9]. Our concierge medicine and Medicare guide explains assignment, the 15% limiting charge and the Advance Beneficiary Notice [9].

Questions people ask

How much does concierge medicine cost?

Published concierge fees we read on October 17, 2026 ran $2,250 to $2,930 a year at three MDVIP physicians, $2,600 to $3,600 a year at PartnerMD and from $4,000 a year at Sollis Health [1][2][3][4]. GAO's 2005 survey found $60 to $15,000 a year [16].

What is the MDVIP membership fee?

MDVIP does not publish a network-wide fee; it says the fee varies by locale and physician [13]. Three MDVIP physician pages we read on October 17, 2026 listed $2,250 (Phoenix), $2,400 (Arlington, Texas) and $2,930 plus a $70 lab charge (Bronxville, New York) [1][11][2].

How much does One Medical cost?

One Medical's membership costs $199 a year and renews until you cancel. It includes 24/7 messaging and video care, while scheduled visits are billed to insurance and copays or deductibles may apply (seen October 17, 2026) [8].

How much does Sollis Health cost?

Sollis Health Signature memberships start at $4,000 a year (seen October 17, 2026). The page lists care coordination, on-site imaging and labs, and priority referrals to 2,000+ specialists [4].

Can I use an HSA for concierge medicine?

Only direct primary care arrangements that meet IRS Notice 2026-5 qualify: fees of no more than $150 a month for one person or $300 for more than one, and no separate billing through insurance. Concierge practices that bill insurance do not qualify [10].

Is concierge medicine tax deductible?

We found no IRS guidance on whether a concierge retainer counts as a deductible medical expense. MDVIP says its fee may be reimbursed through an FSA after the wellness services are received, which is MDVIP's own position [25]. Ask a tax professional.

Does Medicare pay the concierge membership fee?

No. Medicare.gov says Medicare doesn't cover membership fees for concierge care, and you pay the full fee set by your contract with the doctor [9].

How current is the Concierge Fee Index?

Every row shows the date we read the fee. All rows in this version were checked on October 17, 2026. Ten come from the practices' own pages; the Huntington Direct Primary Care fee ($50 to $115 a month by age) comes from its DPC Frontier listing, not the practice's site [6].
