How we check

Executive physical cost: Mayo, Cleveland Clinic and others

The only published executive health price in our research is Human Longevity Inc's Executive Health program at $8,000 a year for people under 65, checked October 17, 2026; Mayo Clinic does not publish a price for its Executive Health Program [1][2]. We could not confirm prices for Cleveland Clinic or other hospital programs, so this page cannot give a typical range.

Medical review: pending. A licensed clinician has not yet reviewed this page.

Executive physical cost is the self-pay price of a bundled preventive evaluation of exams, tests and consults, often completed in one to three days [2].

Annual physical cost without insurance

We did not find a published cash price for a standard annual physical in our research, so we cannot quote one. Membership options include the visit in a monthly fee: direct primary care generally runs $50 to $100 a month per AAFP [15]. One Medical's $199 yearly membership does not include visits, which are billed to insurance [16]. See our direct primary care guide.

Hidden costs

  • Follow-up on findings. In one early Weill Cornell whole-body MRI series, about 30% of people needed follow-up [17]. Ask who pays.
  • Add-ons. Mayo Clinic Scottsdale lists optional add-ons including predictive genomics and multi-cancer early detection [18].
  • Renewal. Human Longevity Inc prices its program per year [1].
  • Travel. Mayo's program runs 1 to 3 days at four locations, so you may also pay for travel and lodging [2].

Insurance, Medicare and HSA

  • Galleri is not covered by Medicare or most insurers [14].
  • Original Medicare generally does not cover a CAC scan [5].
  • The IRS direct primary care HSA rule applies only to primary care arrangements with a fixed monthly fee of no more than $150 for one person, and it excludes labs not typically given in ambulatory primary care [19]. We found no IRS guidance on executive physicals specifically.
  • Ask the program which parts it bills to insurance before you book.

Screening limits

The American College of Radiology says there is not enough evidence to recommend whole-body screening for people without symptoms, risk factors or family history [20].

Executive physical vs concierge vs build your own

A one-time program packs testing into a day or a few days. A concierge membership spreads an annual assessment and doctor access over the year for $2,250 to $3,600 at the practices we read [3][4]. Building your own lets you pick tests and see each price, but you coordinate the results. Read is concierge medicine worth it and our longevity testing guide before you choose.
