Bioidentical: approved versus compounded
A National Academies review commissioned by FDA found insufficient evidence for the clinical utility of compounded bioidentical hormone therapy and said use should be limited to patients who are allergic to an ingredient in an approved product or need a dose form that is not available [7] [8]. Compounded hormones are not FDA-approved; FDA does not review them for safety, effectiveness or quality [19]. TRT clinics: the 300 ng/dL line
Before a Los Angeles TRT clinic prescribes, expect two blood tests. The AUA supports a diagnosis when total testosterone is below 300 ng/dL on repeat testing, together with symptoms or signs [13]. Labels changed in 2025 and 2026
FDA approved the first updated menopause hormone labels on February 12, 2026, removing boxed-warning language on cardiovascular disease, breast cancer and probable dementia [6]. One boxed warning was kept: endometrial cancer, on systemic estrogen-alone products [5].
