Lower obesity, not lower diabetes

San Jose's adult obesity rate is 23.7% in CDC estimates, lower than many large cities, yet 11.1% of adults have diagnosed diabetes [4]. Santa Clara County figures are 21.9% and 10.5% [6]. Obesity rates alone do not capture metabolic risk, so a prescriber looks at blood sugar, blood pressure and history, not BMI only.

Semaglutide or tirzepatide

In SURMOUNT-5, a head-to-head trial, adults with obesity lost an average of 20.2% of body weight on tirzepatide and 13.7% on semaglutide at 72 weeks [7]. Semaglutide is sold as Wegovy (pen or pill) for weight [8]; tirzepatide as Zepbound [9]. Read Wegovy vs Zepbound, semaglutide and tirzepatide.

Insurance in a high-income city

San Jose's median household income, $146,427, is among the highest of the cities we cover [5]. Many residents have employer plans, but Lilly, citing an IFEBP survey, says about half of people with employer insurance lack coverage for obesity drugs [10]. If your plan covers Zepbound, the savings card can cut the cost to as little as $25 [11]; Wegovy's offer for covered patients is as little as $25 a month [1]. Medi-Cal still covers members under 21 [3]. More in insurance and Medicare coverage.

Where to get care

Stanford Health Care serves Santa Clara County; we did not confirm a weight program price there. Prescribers should be licensed by the Medical Board of California [12]. Neither compounded semaglutide nor compounded tirzepatide is FDA-approved, and FDA does not review either for safety, effectiveness or quality [13]. Use the clinic checklist. Nearby: San Francisco, Sacramento and Los Angeles.

Label safety

Both drugs list pancreatitis; get care for severe stomach pain that will not go away [8] [14]. Low blood sugar is more likely if you also use insulin or a sulfonylurea. Facial swelling or trouble breathing calls for urgent help. Each carries a boxed warning about thyroid C-cell tumors and should not be used with a personal or family history of medullary thyroid carcinoma or MEN 2. Stop when pregnancy is recognized. Call 911 for emergencies.
