Concierge and longevity medicine in St. Louis

Missouri is one of the states with a specific law on membership-style primary care: Mo. Rev. Stat. 376.1800, passed in 2015, covers medical retainer agreements [1]. St. Louis practices set their own fees, and national benchmarks seen Oct 17, 2026 run about $2,250 to $3,600 a year for concierge care [2] [3].

Missouri's retainer agreement law

Missouri addressed membership medicine in statute with HB 769 in 2015, codified at section 376.1800 on medical retainer agreements [1]. DPC Frontier lists Missouri among 32 states with a law that places direct primary care outside insurance regulation [6]. For a patient, that means a St. Louis direct primary care practice can charge a periodic fee without being treated as an insurer. Concierge practices that also bill insurance are a different model, as the AAFP explains [7].

St. Louis's median household income of $56,160 [4] is lower than in most cities with large concierge markets. At that income, a $75-a-month DPC plan ($900 a year) is a very different decision from a $2,600 concierge retainer.

Seniors, Medicare and concierge care

With 15.5% of St. Louis residents 65 or older [4], Medicare rules shape many choices. Medicare does not pay the membership fee, and a doctor who accepts assignment cannot charge extra through the fee for Medicare-covered services [5]. MDVIP says its fee pays for an annual wellness program of screenings not typically covered by insurance, while members keep Medicare for other care [10]. HSA rules for 2026

From January 1, 2026, a direct primary care arrangement of up to $150 a month for one person, or $300 for more than one, can be paid from an HSA without ending HSA eligibility [11]. The arrangement must use primary care practitioners, charge a fixed periodic fee as its only payment, and not bill insurance separately [11].
