Menopause weight gain is weight gain and a shift of fat toward the abdomen around menopause, which is confirmed after 12 months without a period.

Why weight changes around menopause
The Menopause Society dates menopause to the final menstrual period, confirmed after 12 months without one, at an average age of 52 [3]. A review by Davis and colleagues in Climacteric concluded that menopause does not itself make women gain weight but affects where weight gain occurs: as estrogen falls, fat moves toward the abdomen [1]. So a steady scale can hide a change in waist size.
That review is from 2012. We did not find a newer Menopause Society or Endocrine Society statement on weight in menopause, so treat it as one well-cited summary, not the last word.
Does HRT help with menopause weight gain?
The evidence we found is thin. In a Mayo Clinic records study presented at ENDO 2025, 120 postmenopausal women on tirzepatide were followed for a median of 18 months. The 40 who also used menopausal hormone therapy lost 17% of total body weight on average, against 14% for the 80 who did not [4]. It was observational, small and not peer-reviewed as presented, so it cannot show that HRT caused the difference.
HRT has its own reasons and risks. See the hormone therapy hub for the 2025 label change and HRT cost for prices.
GLP-1 drugs: what the trials found and what they cost
| Drug | Trial result (average) | Self-pay price | Checked | Source |
|---|---|---|---|---|
| Zepbound (tirzepatide) | SURMOUNT-1: 20.9% loss at 72 weeks on 15 mg; 3.1% on placebo [2] | $299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg, refilled within 45 days) a month on LillyDirect | Oct 17, 2026 | Eli Lilly [6] |
| Wegovy pen (semaglutide) | STEP 1: 14.9% loss at 68 weeks; 2.4% on placebo [5] | $349 a month; $199 for the first 2 fills of 0.25 and 0.5 mg through Dec 31, 2026 | Oct 17, 2026 | NovoCare [7] |
| Wegovy pill (oral semaglutide) | Not in this table; see the Wegovy pill page | $149 (1.5 mg), $199 (4 mg), $299 (9 and 25 mg) a month | Oct 17, 2026 | NovoCare [7] |
Zepbound is approved, with diet and activity, for adults with obesity, or overweight with at least one weight-related condition [8]. Some of the weight lost on these drugs is muscle: in a SURMOUNT-1 body-composition substudy, fat mass fell 33.9% and lean mass 10.9% at 72 weeks, so about a quarter of the loss was lean tissue [9]. See how to keep muscle on a GLP-1.
Stopping matters too. In the STEP 1 extension, people who stopped semaglutide regained about two-thirds of the weight they had lost within a year [10].
Using HRT and a GLP-1 together
Several menopause telehealth clinics now offer both. Midi Health care plans may include HRT and GLP-1s [11]. Evernow prescribes estradiol and progesterone along with brand Ozempic, Wegovy, Zepbound and Mounjaro [12]. Alloy sells brand and compounded GLP-1s [13]; compounded GLP-1s are not FDA-approved, and FDA does not review them for safety, effectiveness or quality [17]. Compare these programs on online HRT providers and the best online GLP-1 programs.
Medicare and GLP-1s for weight
The Medicare GLP-1 Bridge covers GLP-1s for weight management from July 1, 2026 through December 31, 2027. It covers Foundayo, Wegovy (injection and tablets) and the Zepbound KwikPen only [14]. Eligible Part D enrollees pay $50 per one-month supply, and eligibility depends on BMI and certain health conditions [15]. Details are on GLP-1 Medicare coverage.
Safety note for GLP-1 drugs
Zepbound and Wegovy carry a boxed warning for thyroid C-cell tumors seen in rodents, and should not be used by anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 [8][16]. Stop and call your prescriber if you have severe stomach pain that will not go away, which can be pancreatitis, or signs of a serious allergic reaction such as swelling of the face or throat [8]. Low blood sugar is more likely if you also take insulin or a sulfonylurea [8]. Wegovy should be stopped at least 2 months before a planned pregnancy [16]. Call 911 in an emergency.

Questions people ask
Does menopause cause weight gain?
A review in Climacteric found that menopause itself does not cause weight gain, but falling estrogen shifts fat toward the abdomen [1].
Does HRT help with menopause weight loss?
The evidence is limited. In one small observational study presented at ENDO 2025, postmenopausal women on tirzepatide who also used hormone therapy lost 17% of body weight on average, against 14% for those who did not [4]. That design cannot prove HRT caused the difference.
Can I take a GLP-1 for menopause weight gain?
GLP-1 drugs such as Zepbound are approved for adults with obesity, or overweight with a weight-related condition [8]. They are not approved for menopause itself. A prescriber decides whether you qualify.
How much weight do people lose on GLP-1 drugs?
How much do GLP-1s cost without insurance?
Does Medicare cover GLP-1s for weight loss?
Will I lose muscle on a GLP-1?
Some. In a SURMOUNT-1 substudy, lean mass fell 10.9% and fat mass 33.9% at 72 weeks, so about a quarter of the weight lost was lean tissue [9].
