Tirzepatide is the active ingredient in Zepbound and Mounjaro, a weekly injection that acts on two gut hormone receptors, GIP and GLP-1.
Zepbound, sleep apnea and Utah Medicaid
When Utah's obesity-only Medicaid pilot ended, one tirzepatide path stayed open: Utah Medicaid covers Zepbound for obstructive sleep apnea with prior authorization [2]. FDA approved that use on December 20, 2024 for adults with obesity [6]. In the label's sleep apnea trials, 42.2% and 50.2% of people on Zepbound reached remission or mild, symptom-free apnea at 52 weeks, versus 15.9% and 14.3% on placebo [7]. With 29.5% of Salt Lake City adults living with obesity [3], ask whether a sleep study makes sense.

A year of cash Zepbound, by dose
| Dose | Per 28-day fill | 13 fills | Ref |
|---|---|---|---|
| 2.5 mg | $299 | $3,887 | [1] |
| 5 mg | $399 | $5,187 | [1] |
| 7.5 to 15 mg, refilled within 45 days | $449 | $5,837 | [1] |
| 10 to 15 mg, refill late | $699 | $9,087 | [1] |
Salt Lake City's median household income is $75,090 [4], so a maintenance year at $449 a fill is close to 8% of it. Refilling late at 10 mg or above pushes the year past $9,000. See tirzepatide costs and the cheapest legal ways to buy tirzepatide.
Vial or KwikPen?
- Vial: needs a syringe that can measure 0.5 or 0.6 mL, with a new syringe and needle every injection [8].
- KwikPen: four weekly doses in one pen, launched February 23, 2026 [9]. The label says it is not recommended for self-injection by people with impaired vision, and it must never be shared [8].
- Medicare: the GLP-1 Bridge covers the Zepbound KwikPen but not vials or single-dose pens [10].
Staying on it versus stopping
SURMOUNT-4 tested what happens after 36 weeks of tirzepatide. People who kept taking it reached an average 25.3% weight reduction by week 88; those switched to placebo ended at 9.9%. And 89.5% of those who continued kept at least 80% of their earlier loss, compared with 16.6% on placebo [11]. That is worth weighing against the yearly cost. See stopping tirzepatide.
Compounded tirzepatide
Compounded tirzepatide is not FDA-approved [12]. FDA ended enforcement discretion for pharmacy compounding of tirzepatide in March 2025, and a federal appeals court upheld FDA's shortage decision on August 27, 2026 [13][14]. Read compounded tirzepatide.
Tirzepatide label warnings
Boxed warning for thyroid C-cell tumors in rats; do not use with a personal or family history of medullary thyroid carcinoma or MEN 2. Stop and seek care for severe stomach pain that persists or an allergic reaction. Oral birth control may work less well for 4 weeks after starting and after each dose increase, so the label advises a backup or non-oral method. Stop when pregnancy is recognized [7][15]. Call 911 in an emergency.

Did Zepbound help sleep apnea in trials?
In the label's 52-week trials, 42.2% and 50.2% on Zepbound reached remission or mild non-symptomatic sleep apnea, compared with 15.9% and 14.3% on placebo [7].
Does Medicare's GLP-1 Bridge cover Zepbound vials?
