Medical weight loss is obesity treatment managed by a licensed prescriber, often with an FDA-approved GLP-1 drug, nutrition care and follow-up.
What changed for Pennsylvania Medicaid
A Pennsylvania Medical Assistance bulletin dated November 24, 2025 ended coverage of GLP-1 drugs for overweight and obesity starting January 1, 2026 [1]. Saxenda was dropped for all uses, other GLP-1 uses need prior authorization, and non-GLP-1 obesity drugs such as phentermine remain covered [1]. KFF lists Pennsylvania among four states that ended this coverage after October 2025 [6]. For a city with a median household income of $61,953 [5], a $349 monthly cash price is a real burden.

Philadelphia's health numbers
Philadelphia County, which is the same area as the city, had 1,574,281 residents in July 2025 [5]. CDC PLACES estimates 34.2% of adults had obesity and 14.6% had diagnosed diabetes in 2022 [4]. These figures are from the 2024 PLACES release because the 2025 release had no 2023 obesity or diabetes rows for Pennsylvania counties when we checked [7]. People with type 2 diabetes may still get a GLP-1 such as Ozempic or Mounjaro covered for blood sugar, since the bulletin only ended obesity coverage [1].

Main Line and city clinics: what to ask
We could not confirm hospital weight program details for Philadelphia during this update, so we do not list any. Whether you are in Center City or on the Main Line, ask:
- Does the Pennsylvania State Board of Medicine show the prescriber's license as active [9]?
- Is the drug a brand product or compounded? Compounded semaglutide and tirzepatide have no FDA approval, and FDA does not check them for safety, effectiveness or quality [10].
- What will a month cost at the dose you expect to reach?
- Are labs part of the fee, and how often are they drawn?
- What happens to months you paid ahead if you quit?
Medicare and employer coverage
Philadelphia retirees on Part D who qualify can get a GLP-1 through the Medicare GLP-1 Bridge for $50 a month until December 31, 2027 [11]. On a job-based plan that already covers Zepbound, Lilly's savings card can drop the cost to $25 [12]. Coverage routes are on GLP-1 insurance and Medicare.
Warning signs
Severe stomach pain that persists may be pancreatitis, and swelling or hives can be an allergic reaction: in either case stop the drug and get care. Insulin or a sulfonylurea taken with a GLP-1 raises the chance of low blood sugar. Animal studies are behind a boxed warning on thyroid C-cell tumors, and the labels say to stop once pregnancy is recognized [13][14]. Call 911 for an emergency.

Questions people ask
Does Pennsylvania Medicaid still cover Wegovy or Zepbound for weight loss?
No. Medical Assistance ended GLP-1 coverage for overweight and obesity on January 1, 2026; non-GLP-1 drugs such as phentermine are still covered [1].
Can Philadelphia Medicaid members with type 2 diabetes still get a GLP-1?
The November 24, 2025 bulletin ended only obesity coverage; other GLP-1 uses need prior authorization [1].
What is the adult obesity rate in Philadelphia?
CDC PLACES estimates 34.2% of Philadelphia County adults had obesity in 2022 and 14.6% had diagnosed diabetes. That is the latest year available for Pennsylvania [4].
Why are Philadelphia obesity figures from 2022 and not 2023?
When we checked the 2025 PLACES release, it had no 2023 obesity or diabetes rows for Pennsylvania counties, so the 2024 release is the newest source [7].
Do Main Line weight loss programs pay less for the drugs?
Where do I check a Philadelphia doctor's license?
The Pennsylvania State Board of Medicine runs the license lookup [9].
