In a concierge arrangement you pay a primary care doctor a retainer for more time and quicker contact, and the practice usually keeps billing your health plan for visits.
Cleveland Clinic's Las Vegas concierge practice
The Las Vegas office is part of Cleveland Clinic Concierge Medicine, which also runs sites in Florida and in Cleveland. Its page promises a physician you can reach around the clock, a same- or next-day slot on most weekdays, a yearly comprehensive assessment, chronic disease care, help lining up specialists, vaccines at cost, and every office or virtual visit with your concierge provider [1]. Because the price is not posted, request it in writing together with the cancellation terms.

Retainers that come with a public price
| Who | Published price | Ref |
|---|---|---|
| Internist in the MDVIP network, Phoenix | $2,250 per year | [2] |
| MDVIP doctor in Bronxville, New York | $2,930 yearly, with a $70 lab charge on top | [3] |
| PartnerMD, all of its locations | $2,600 to $3,600 annually | [4] |
| Direct primary care, per the AAFP | $50 to $100 monthly | [7] |
Set against the city's $73,877 median household income [5], the $2,250 Phoenix retainer is roughly 3% of a year's earnings, before any premium. MDVIP says each doctor's fee depends on place and physician, and members can usually split it into quarterly or half-year payments [8].
Direct primary care with no Nevada statute
DPC Frontier finds no Nevada law that carves direct primary care out of insurance regulation [6]. The model itself is a flat monthly charge, usually $50 to $100, with no insurance claims [7]. For HSA holders, federal rules changed in January 2026: a qualifying plan of up to $150 a month for one person, or $300 for a family, can be paid from the account without ending eligibility [9] [10]. A retainer practice that also bills your plan sits outside that safe harbor [9]. Background: direct primary care and concierge vs DPC.
Retirees and the retainer
About one Las Vegas resident in six is 65 or older (16.0%) [5]. For them the retainer is an out-of-pocket cost, because Medicare pays none of it, and a doctor who takes assignment cannot fold Medicare-covered care into the fee [11]. Our Medicare guide for concierge patients explains the Advance Beneficiary Notice.
