Concierge medicine is primary care with an annual or monthly membership fee that pays for extra access and preventive services.
Why the market looks different here
Bergen County is our reference county for New Jersey, and the local figures here come from it. Bergen County's median household income is $124,884, and 19.8% of residents are 65 or older [2]. That combination, high income and an older population, is the profile concierge practices target. We found no New Jersey practice that lists its fee, so most comparisons start with a phone call.

New Jersey law and HSAs
DPC Frontier lists no direct primary care statute in New Jersey [3], so DPC practices here operate without a state law defining them. Federal HSA rules apply anyway: since January 1, 2026, DPC fees up to $150 a month for one person can be paid from an HSA without losing eligibility, but arrangements that bill insurance separately are excluded [9]. Compare the models in concierge vs DPC.
Medicare rules for a 65+ population
With nearly a fifth of Bergen residents over 65 [2], Medicare rules matter. Medicare does not cover membership fees. A doctor who accepts assignment cannot charge extra for covered services, and must give an Advance Beneficiary Notice for noncovered items included in the fee [10]. Details: concierge medicine and Medicare.

Neko Health opened its first US clinic in SoHo in September 2026 with a $499 scan [13]. The American College of Radiology does not recommend total-body screening for people without symptoms [14].
