The DC patient profile

Washington DC is younger than many cities: 13.6% of residents are 65 or older [2]. Its median household income, $109,870 [2], means many residents can afford cash-pay hormone clinics, which helps explain the number of bioidentical and pellet offers in the area. Higher prices do not mean better evidence, so start with what the labels and guidelines say.

Guideline basics in five points

  1. Menopause is confirmed after 12 months without a period, at an average age of 52 [5].
  2. FDA's updated labeling recommends starting systemic HRT within 10 years of menopause onset or before age 60 [6].
  3. The boxed warning for endometrial cancer remains on systemic estrogen-alone products [6].
  4. Testosterone needs two low tests, below 300 ng/dL, plus symptoms [7].
  5. Testosterone is not approved for age-related low testosterone alone [8].

Pellets and compounded creams

Compounded bioidentical hormones are not FDA-approved, and FDA does not review them for safety, effectiveness or quality [18]. The National Academies found insufficient evidence for their clinical use and recommended FDA consider adding pellets to its Difficult to Compound list [14]. The Menopause Society also cites safety concerns [15]. See hormone pellet cost.

Safety reminders

Testosterone can raise blood pressure, and FDA added that warning to all products in 2025 [16]. In TRAVERSE, more men on testosterone had pulmonary embolism (0.9% vs 0.5%) [17]. Keep gels away from children [8]. Call 911 for chest pain, sudden shortness of breath or stroke signs.
