Insurance and Texas Medicaid
Obesity treatment with a GLP-1 is not a Texas Medicaid benefit, per KFF [5]. Wegovy, however, has carried a second, cardiovascular indication since March 8, 2024, for adults with established heart disease plus overweight or obesity [11]; a commercial plan may treat that use differently, so ask. Where an employer plan does cover Wegovy, Novo's copay offer can take the monthly cost down to $25 [1].
Compounding pharmacies in Texas
Any San Antonio pharmacy that compounds answers to the Texas State Board of Pharmacy [4]. Under FDA's current policy, a traditional compounding pharmacy may make semaglutide only for an individual patient whose prescriber records a clinically significant reason the approved product will not do; large outsourcing facilities are shut out of using bulk semaglutide altogether [12]. The 68-week STEP 1 trial is the yardstick: 14.9% average weight reduction on the 2.4 mg dose against 2.4% for placebo. Gastrointestinal trouble made 4.5% of the semaglutide group quit, compared with 0.8% given placebo [14].
Know these label warnings
The thyroid boxed warning (C-cell tumors in rodent studies) rules semaglutide out for anyone with medullary thyroid carcinoma or MEN 2 in their personal or family history. Unrelenting stomach pain and allergic reactions both call for stopping and getting care. Paired with insulin or a sulfonylurea, lows become more likely. Pregnancy plans mean stopping two months ahead, per the label [15]. 911 is for emergencies.
