Concierge medicine is a membership arrangement in which a primary care practice charges a set yearly fee for extended access, longer visits and a smaller patient load.
An in-state price to start from
PartnerMD lists offices in the Atlanta area, Virginia, Maryland and Greenville, South Carolina, and gives a general membership cost of $2,600 to $3,600 a year, with monthly, quarterly, semi-annual or annual billing and 30 days' notice to cancel with a prorated refund [1]. Its location prices sit behind a calculator, so a Greenville quote may differ. On Charleston's $92,414 median household income [2], that range is about 2.8% to 3.9%.

South Carolina and direct primary care
DPC Frontier says South Carolina has no DPC law but mentions a 2018 bill, H 4643, without saying whether it passed, so the status is unclear [3]. Direct primary care can operate either way. Since January 1, 2026, HSA owners can pay qualifying DPC fees of up to $150 a month for one person from their HSA, but concierge practices that bill insurance do not qualify for that rule [7]. See direct primary care and concierge vs DPC.
Local health systems
Charleston's large systems are MUSC Health and Roper St. Francis Healthcare. We did not confirm a concierge or executive health program at either. If you want a one-time deep checkup instead of membership, see executive physicals.
Medicare in a retiree-friendly city
About 16.4% of Charleston residents are 65 or older [2]. Medicare does not cover concierge membership fees, and if the fee covers services Medicare does not pay for, the doctor must give you an Advance Beneficiary Notice [8].
