Concierge medicine is primary care sold with a yearly retainer that buys extra access, while the doctor usually still bills your insurer for visits.
Three models sold under one label
The GAO defines concierge care as a practice that charges patients a membership fee for enhanced services or amenities [6]. The AAFP separates the first two: a concierge contract is yearly, costs more and may still go to insurance, while DPC is a lower monthly charge that generally skips insurance [7]. Longevity clinics sell advanced testing. On a $64,813 median household income [2], the $2,400 Arlington retainer equals about 3.7% of a typical Houston household's year, which is why the cheaper DPC model deserves a look.

Texas DPC law and HSA money
Texas put direct primary care in its Occupations Code, chapter 162, subchapter F, per DPC Frontier's state summary [3]. On the federal side, HSA owners gained a new option in January 2026: a DPC plan of $150 a month or less for one person no longer blocks contributions and can be paid from the account [10]. A concierge office that files insurance claims on top of its retainer does not count [10].
Medicare in Houston
Roughly one Houston resident in eight, 12.3%, is 65 or older [2]. Medicare pays nothing toward a retainer; if the doctor accepts assignment, the fee cannot cover Medicare services, and any non-covered extras it buys must be listed on an Advance Beneficiary Notice [11]. The rules are spelled out in concierge medicine and Medicare.
Scans and executive physicals
Prenuvo has a Houston center [4]; it charges $2,499 for the Whole Body scan and $1,199 for the Focused scan [5]. The American College of Radiology says the evidence does not support total-body screening in people without symptoms or risk factors [12]. Most patients pay $100 to $150 cash for a coronary calcium scan, according to GoodRx [13]. Mayo Clinic's Executive Health Program operates in Rochester, Scottsdale, Jacksonville and London, with no listed price [14]. What to ask a Houston practice
- What is the yearly fee, and can it be paid monthly or by the quarter?
- Which services sit inside the fee, and which go to my insurer?
- How large is each doctor's patient panel?
- What refund do I get if I cancel early?
- Is the doctor in good standing with the Texas Medical Board [15]?
Ask the same questions in The Woodlands, Sugar Land and other suburbs. Our concierge doctor finder guide explains each one. Other Texas pages: Dallas, Austin and San Antonio. Start at the concierge medicine guide.
