What Medical Assistance still pays for

Pennsylvania's November 2025 bulletin did three things from January 1, 2026: it ended GLP-1 coverage for overweight and obesity, removed Saxenda for all indications, and required prior authorization for other GLP-1 uses. Non-GLP-1 obesity drugs such as phentermine remain covered [2]. If you have type 2 diabetes and Medical Assistance, ask your prescriber to file the prior authorization for Ozempic, which is approved for diabetes [6].

If you are offered compounded semaglutide

If you are offered it, know that compounded semaglutide is not FDA-approved and that FDA does not review compounded drugs for safety, effectiveness or quality [11]. FDA says 503B outsourcing facilities cannot compound semaglutide from bulk ingredients because it is neither on the 503B bulks list nor in shortage [8]. FDA has warned telehealth firms against calling compounded drugs "generic" or "the same as" an approved drug [9]. See compounded semaglutide.

Watch for these

Get care for severe stomach pain that does not stop, which can be pancreatitis [10] [6]. Semaglutide has a boxed warning about thyroid C-cell tumors and is not for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2. Hypoglycemia becomes more likely when semaglutide is paired with insulin or a sulfonylurea. Seek urgent help for facial swelling or trouble breathing. If you become pregnant, the Wegovy label says to discontinue it. Call 911 in an emergency.
